The understanding of hemostasis physiology has been considerably advanced by models of kinetics and by the complicated interplay of cells and soluble coagulation factors. How this physiology is currently, or will eventually, be reflected in clinical laboratory testing is the subject of this monograph; this information is key for laboratorians to implement physiologic concepts into practical data and for clinicians to understand the basis of and therefore correctly treat hemostatic disease. Many thousands of patients present with bleeding disorders or bleeding complications of other diseases, and millions of individuals suffer the morbidity and mortality of thromboembolic complications. How laboratory testing confronts the challenge of predicting different hemostatic risks and guiding therapy is the critical subtext of the chapters in this monograph. New developments in hemostasis physiology have identified thrombin as an important, if not central, coordinator of hemostatic function and thus a target for measurement to assess hemostatic function and risk. Whether such measurements as endogenous thrombin potential or thromboelastography will accurately predict and/or quantitate global hemostatic function is an important question. Modeling the clinical risks of bleeding or thromboembolism currently uses the laboratory presence or absence of particular risk factors, but our clinical understanding of risk appears to more closely approximate a dynamic model, even within individual patients. Therefore, testing for platelet dysfunction or comprehending the functional implications of a lupus anticoagulant may rely on our evolving comprehension of hemostasis phyisology and perhaps require more sophisticated interpretation of clinical predictors of hemostatic risk. This monograph aims to shed some light in these areas and promote investigation of such key hemostasis issues.

Eric Preston, Emeritus Professsor of Haematology, University of Sheffield and Director, WHO and WFH International External Quality Assessment Programs for Blood Coagulations, Sheffield, UK The hemostasis laboratory has a vital role in the diagnosis and management of patients with familial and acquired hemorrhagic and thrombotic disorders. Rapid changes in the number and complexity of tests in this discipline have presented challenges for laboratories, as they develop quality programs for the oversight of this testing. Quality in Laboratory Hemostasis and Thrombosis has been completely revised and updated to reflect the changing process of managing quality. The second edition provides information on all aspects of testing, from pre-analytic to analytic and result reporting, in addition to external quality assurance. Chapters throughout the book include the development of global guidelines for methods, as well as the preparation of international standard plasmas and reagents. Designed to capture the elements of quality at all levels of the practice of laboratory hemostasis and thrombosis, this book will guide the reader through the development of a quality program to support all activities in the hemostasis laboratory, both simple and complex. All anesthesiologists are confronted with patients who show bleeding disorders, whether congenital, acquired, or pharmacologically induced. Although many studies, meta-analyses, guidelines, and textbooks have been published on the subject, they mostly cover specific aspects or require a thorough knowledge of hemostasis. The goal of this book is to provide the anesthesiologist with an overview of hemostasis and the mechanisms underlying bleeding and coagulation in general and to assist in the understanding of specific coagulation disorders as they may occur in the various anesthesia subspecialties. It is hoped that, through the provision of practical information and tools, the book will help residents and trained anesthesiologists to manage one of the most frustrating challenges that they face: the bleeding patient. A full understanding of coagulation requires a lifelong career, and this book is not intended to replace the consulting hematologist, whose expert opinion should always be sought. The available procoagulant blood products and drugs all have potentially dangerous side-effects; furthermore, in bleeding disorders pitfalls are frequent and a misdiagnosis can have potentially catastrophic consequences. The aim of the editors is thus instead to enhance the collaboration between the disciplines of Anesthesiology and Hematology.
