Even the best hernia repair can result in postoperative difficulties for the patient caused by repair sequelae as for example pain, infertility, infection, adhesion and dislocation of the protheses. That can happen many years later and now, where the general principle of hernia repair is well understood all over the world, these sequelae are noticed more and more. To define them, to evaluate the absolute and relative risk of these sequelaes and to describe the ways of their prevention, diagnosis and treatment, the 5th Suvretta meeting had focussed on this subject. We discussed if there’s a principle risk by technique, material or both. The results of these discussions and the future handling and evaluation of this problem was the aim of this meeting. Even the best method can be made better by optimization of its single components. The repair and management of inguinal hernias represents a significant part of the general surgeon's workload. It was therefore inevitable that following the success of laparoscopic cholysysectomy, surgeons would develop a procedure for repairing inguinal hernias laparoscopically. This book provides the first comprehensive account of laparoscopic inguinal hernia repair. The Editors' aim has been to give step-by-step guidance to each operative procedure discussed with reference to long term results. One of the biggest problems facing surgeons learning new laparoscopic procedures is an understanding and orientation of basic anatomy when viewed through the laparoscope. This issue is discussed at length with guidance on how to avoid the common pitfalls. Final chapters look at the advantages and disadvantages of the Lichtenstein open mesh hernia repair approach, safety and properties of non-absorbal mesh with the last chapters concentrating on laparoscopic suture repair and the new mini hernia operation.

Patients affected with a hernia continue to represent a significant clinical population. Resulting from varied causes and associated with an array of pathophysiologies, a hernia can pose marked diagnostic and treatment challenges for the clinician. Sometimes subclinical and sometimes life-threatening, a hernia may require little or substantial medical intervention. This book consists of sixteen chapters by noted clinicians and scientists. Many chapters include background pathogenesis, typical symptoms experienced by patients, diagnostic measures, and therapeutic approaches. Current information is summarized and interpretations offered where appropriate.Basic pathogenic mechanisms and pathophysiology are considered in chapters such as those on the inheritance of a groin hernia (Burcharth and Rosenberg), and on alterations in arterial blood gases both before and after surgery (Tsounaki). These chapters make evident both the progress made in understanding the processes underlying hernias and related diseases, as well as the incomplete understanding we have with respect to such processes.Descriptions of clinical presentations, diagnostic approaches, and medical and surgical management are provided for several types of hernias, including inguinal, diaphragmatic, incisional, and Richter's. The utility of new methods for correction, such as laparoscopy and components separation, is described in the chapters by Maxwell and Baird, Marescaux et al, Mazzochi et al, and Sabino et al. These chapters reflect the technological improvements that have been brought to the successful treatment of hernias. It is likely that, with time, further definitions and refinements will additionally advance the understanding and approaches to hernias. The goal of this book is to summarize our understanding of basic concepts of hernia, at present, as a reference for scientists and clinicians.

This text is designed to present a comprehensive state-of-the-art approach to options available for inguinal hernia repair. Early chapters address anatomic evaluation of the groin, preoperative optimization of outcomes, and considerations in choosing a surgical technique. It then transitions to the clinical management of this common medical condition, specifically focusing on operative details. Written by experts in the field of hernia repair, the clinical chapters detail both open and minimally invasive techniques for repair including detailed anatomic drawings, surgical photos, and links to videos of operative techniques. Postoperative management is explained, detailing the common complications and addressing the importance of surgical outcomes, especially in the setting of “pay for performance” metrics. The final section of the text concludes with the management of inguinal hernias in select situations, including children, those performed with concomitant procedures, and modifications for training residents in this common surgical procedure. Each chapter includes a review of the published literature and selected references, along with anatomic illustrations, videos from operative interventions, and surgical photos to help reinforce the text. Surgical Principles in Inguinal Hernia Repair: A Comprehensive Guide to Anatomy and Operative Techniques will serve as a comprehensive resource for surgeons on patient preparation, surgical techniques, and outcomes for the management of inguinal hernias.

Laparo Endoscopic Hernia Surgery written by Reinhard Bittner and has been published by Springer this book supported file pdf, txt, epub, kindle and other format this book has been release on 2018-06-25 with Medical categories.

This book is distinctive in that it focuses exclusively on current laparoscopic and endoscopic techniques for inguinal, primary and incisional abdominal wall, and hiatal hernias. Individual steps in diagnosis and treatment are described by experts in the field, but this clinical expertise is also integrated with the best available external evidence from systematic research as encapsulated in statements, recommendations, and guidelines. The reader will thus not only learn how to perform techniques systematically and reproducibly but also come to understand which of the procedures have been scientifically validated by studies, reviews, and meta-analyses and which have simply developed empirically. The descriptions of technique are supplemented by detailed guidance on such aspects as indications, anesthesia, aftercare and pain management, and the prevention and management of complications. Where appropriate, careful comparisons are made of competing repair options, including open techniques. In summary, this book will help practicing surgeons to standardize their operative technique so as to reflect current scientific knowledge and thereby improve the quality of laparoscopic/endoscopic hernia surgery.

The enthusiastic feedback received from readers of the international bestseller Schein's Common Sense Emergency Abdominal Surgery (now in its 3rd edition, translated to seven languages; Springer Verlag; ISBN 9783540748205) inspired the editors to do a similar book dedicated to surgical complications: practical, non-formal, internationally relevant (in all types of practice and levels of hospitals) – and definitely not politically correct: what is considered taboo by others is not taboo for us; here we discuss everything! As in the editors’ previous book, the use of references is restricted to the absolute minimum, and citing figures and percentages is avoided as much as possible. The chapters in this book are the opinion of experts – each contributor has a vast personal knowledge and clinical experience in the field he is writing about. This book will help all surgeons (and their patients), avoid the misery of complications, and will provide advice on the management of those that are unavoidable. Complications and death are an integral component of surgery. Surgeons have to look death in the eyes, try to prevent it and vanquish it – this is what this book is all about. "Schein’s books are always terrific reads, conveying pithy observations and practical advice and doing so with wit and scholarship. This latest is surely a must for every surgeon’s bookshelf." Abraham Verghese MD, author of CUTTING FOR STONE
