In the last 10 years, total prosthetic replacement has become the hip oper ation for many but not all surgeons. In our clinic the increasing frequency of TP has been paralleled by a reduction in the frequency of 10 and HA. For the experienced orthopaedic surgeon and hip surgeon, hip disease has so many facets that it could not possibly be tackled with one operation alone. Thus, HA seems recently to have won renewed recognition. It has once more become the procedure of choice in specific cases, possibly because of certain disadvantages and complications associated with TP which have somewhat tempered the original optimism. Currently, the risk of infection or loosening of the pros thesis is such that the operation cannot justifiably be recommended for patients under 55-60 years of age. In contrast with HA, in the majority of cases the results of TP or 10 are not fully predictable. The loss of joint motion is offset by two important advantages: -complete freedom from pain - full weight bearing is possible on the arthrodesed hip. On following up our HA operations from 1961 to 1971, we examined the development of the operative procedures in our clinic. We found that the range of indications had narrowed as stricter criteria were applied in choosing patients for operation. Adequate and precise operative tech niques were developed. We studied the biomechanics of the hip joint exten sively (effect of arthrodesis on the statics and dynamics of the hip joint and neighbouring joints).

This open access book describes and illustrates the surgical techniques, implants, and technologies used for the purpose of personalized implantation of hip and knee components. This new and flourishing treatment philosophy offers important benefits over conventional systematic techniques, including component positioning appropriate to individual anatomy, improved surgical reproducibility and prosthetic performance, and a reduction in complications. The techniques described in the book aim to reproduce patients’ native anatomy and physiological joint laxity, thereby improving the prosthetic hip/knee kinematics and functional outcomes in the quest of the forgotten joint. They include kinematically aligned total knee/total hip arthroplasty, partial knee replacement, and hip resurfacing. The relevance of available and emerging technological tools for these personalized approaches is also explained, with coverage of, for example, robotics, computer-assisted surgery, and augmented reality. Contributions from surgeons who are considered world leaders in diverse fields of this novel surgical philosophy make this open access book will invaluable to a wide readership, from trainees at all levels to consultants practicing lower limb surgery

When dozens of holes appeared in a patient's femur alongside his hip prosthesis, experts were baffled as such a phenomenon had never been seen before. Over the first four decades of total hip surgery, the severe bone destruction multiplied, crippling many thousands of patients. Eventually identified as periprosthetic osteolysis, this devastating disease affected over 1 million patients and was the leading cause of failure in total hip surgery. While total hip surgery dramatically reversed severe arthritis of the hip, the same operation simultaneously created a relentless 'particle generator' in the body. Ironically, in the effort to do good, doctors were simultaneously doing major harm. Vanishing Bone: Conquering a Stealth Disease Caused by Total Hip Replacements takes readers on a detective adventure in contemporary medical science, from the identification of the cause of the disease through the complex process of affecting its cure. Dr. William H. Harris and his colleagues played an important role in solving the mystery of this disease, pointing to its molecular biology, recognizing the unusual wear of the prostheses, and ultimately developing a new material for use in the manufacture of hip implants. With more than 6 million people walking on this stable, low-wear material, the disease has been virtually eliminated among those with total hip replacements worldwide. Diseases are rarely observed, revealed, and eliminated in one lifetime. Vanishing Bone tells the captivating story of one such disease through an engaging account of scientific and medical innovation.

Postel, Kerboul, Evrard, Courpied, and their coauthors take a completely objective attitude in describing the progress achieved in total hip replace ment with reference to their own experience over the last 20 years. They avoid any triumphant fanfares, but not because of Pascal's dictum: "Do you want people to speak well of you? Don't do it yourself. " Rather, they know that other surgeons, like themselves, are more concerned with effi ciency than with laurels, and want that is, new ideas based on sufficiently wide experience and analyzed in a strict and uncompromising manner. In addition, surgeons are particularly anxious for in-depth study of the pr,ob lems, complications, and failures encountered as well as for indications as to how these can be avoided and corrected. This book is sure to satisfy sur geons on both these counts. It is the exciting and almost incredible prod uct of the work and the immense progress that have taken place in the 15 years since I retired. The spirit behind this work has inspired it from the start. It is characterized by a determination to take constant study of the results as the only guide in matters of indications and technique, and the authors insisted on a system of documentation whose purpose (and perhaps merit) was to facilitate comparison both of the preoperative functional state and of the final re sult; they have also kept an open mind for interesting new insights from whatever quarter they might arise.
