The Neurology of HIV Infection covers all aspects of nervous system involvement and pathology in HIV-infected individuals. Specialists in this field cover epidemiology, global aspects, pathology and pathogenesis of nervous system disease in HIV-infection. All complications, including the pathology caused by HIV itself and all opportunistic infections of the nervous system are reviewed in detail. Both central nervous and peripheral nervous system complications, including neuropathies and myopathies, are discussed. Key chapters on global developments, HIV-associated neurocognitive disorders, IRIS, stroke and neuro-aids in children complete this volume. Covers all aspects of nervous system involvement and pathology in HIV-infected individuals Includes the pathology caused by HIV and all opportunistic infections of the nervous system Presents key chapters that focus on global developments, HIV-associated neurocognitive disorders, IRIS, stroke and neuro-aids in children

The human immunodeficiency virus (HIV) commonly enters the central nervous system, disturbs brain function, and ultimately results in serious brain damage. The most dangerous manifestation of this infection of the brain is HIV-associated dementia, which includes extreme memory loss, slowness in thinking, disorientation, and social incapacity. Most persons who are HIV-infected do not develop severe dementia; however, perhaps 50% of those with AIDS and a smaller proportion of HIV-infected carriers suffer from more subtle brain disorders that can be revealed by proper neuropsychological testing. This ground-breaking volume is the first to summarize the current state of knowledge about the neurocognitive disorders associated with HIV-1 infection. With contributions from leading authorities in the field, the book fully discusses the prevalence, qualitative features, natural course, and effects of neuropsychological impairments in persons with HIV infection. Neuropsychological data are related to findings from studies of brain imaging, neuropathology, and the effects of antiretroviral treatments. This critical work will be essential for neuropsychologists, infectious disease physicians, neurologists, and all those involved in AIDS research and treatment.

During the past two decades, the world scientific community has witnessed major achievements in our understanding of the pathogenesis of HIV infection of the nervous system and HIV-Associated Dementia (HAD). Despite these giant gains, nervous system involvement during AIDS remains a relentlessly progressive disease with a deadly fate in many cases. This book on NeuroAIDS provides a unique resource for both general neurologists as well as basic neuroscientists with profound interests for research on NeuroAIDS. This book has special emphasis on the mechanisms of disease development and progression of HIV-infected patients with NeuroAIDS. The contributors have provided the readers with comprehensive reviews on clinical manifestations of HAD, mechanisms of HIV entry into the central nervous system, the role of cytokines and chemokines in pathogenesis of NeuroAIDS, drug abuse and NeuroAIDS, virus load in HAD, allostasis in HIV and AIDS, stroke in AIDS patients, and neuroimaging of HIV infection of the central nervous system. In addition, there are chapters on Varicella Zoster virus infection of HIV-seropositive and AIDS patients, as well as the molecular basis for opioids and AIDS virus interactions.

In this exciting symposium, the editor brings to print important new information on AIDS and how HIV affects the brain. Each chapter focuses on one or more of the cell types that reside in or traffic through the central nervous system (CNS). Each of these cells is important to considerations of the pathogenesis of the CNS. Neurologists, AIDS physicians, and other professionals caring for AIDS patients will find that this “cell-based” view provides a unique perspective and that it will guide and stimulate future investigation of this clinically important and pathogenetically intriguing disorder. The editor also introduces some general considerations for therapeutic intervention of AIDS dementia complex (ADC). The contributors to The Cellular Basis of Central Nervous System HIV-1 Infection and the AIDS Dementia Complex deal with the cells and mechanisms involved in HIV-1 brain infection and the resultant ADC. Each author was asked to review the involvement of their assigned cells in CNS HIV-1 infection and how these cells might be involved in the pathology and process of brain injury associated with ADC. Readers will be enlightened on the functional roles of various cells and how these cells and mechanisms might fit into the broader picture of ADC pathogenesis.

The National Institute of Mental Health (NIMH) AIDS Program is the fourth largest acquired immune deficiency syndrome (AIDS) program within the National Institutes of Health (NIH). Since 1983, our program's contributions have concentrated on two major areas. The first has been to develop effective strategies to prevent or reduce behaviors that place individuals at risk for human immunodeficiency virus type 1 (HIV-1) infection. The second has been to support and foster research to enhance our understanding of the profound impact of H IV-1 infection on the central nervous system (eNS). The brain appears to be a prime target of the virus and may serve as a reservoir for the virus. Post mortem examination of brain tissue has provided evidence of eNS cell damage in 80%-90% of people who die with AIDS. For about 10%-20% of people with AI DS, mild neuropsychological symptoms are the first signs of the disease. Approximately 20% of individuals infected with HIV-1 develop AIDS dementia complex. The symptoms include apathy, difficulty concentrat ing, irritability, depression, and personality changes. In the later stages of the disease, people may experience psychiatric disor ders. Death usually occurs within 6 months of the appearance of those more severe symptoms. Up to 90% of children infected with H IV-1 experience attention and concentration difficulties and often experience neurodevelopmental delay or regression over time.
