We are more likely to be lonely tomorrow as a result of the freedoms we enjoy today. The risk of loneliness in old age is on the rise due to changing ways of life in Britain. Our families are more fragmented, more people are childless and we are living longer, which all increase our chances of being socially isolated in later life. A growing number of older people can be expected to be housebound with minimum social contact. Many of these risk factors are associated with social progress and independence, particularly for younger people. But they make it harder for us to sustain the social networks on which we may have to rely for support as we grow older. Policy-makers have tried to protect older people against isolation through efforts to integrate health and social care services, and to involve older users more actively in making decisions about their own care. Yet more must be done to improve the quality of life of those living housebound lives. Over 30 people die alone and unnoticed in their own homes each year, and one in six older people living alone rate themselves as ‘often or always lonely’.Unless significant changes occur, by 2021 nearly 2.2 million over-65s will be socially isolated. The benefits of promoting independent living for older people are enormous, for individuals, our public services and wider society. Extending years of active life will allow older people to play a full role in family life and in their local communities. Home Alone argues that users need to become ‘co-producers’ of personalised services. Policy-makers must learn from the voluntary and community sectors to help older housebound people develop their own networks of support.

This report examines the response to SARS by public health systems in individual countries, the biology of the SARS coronavirus and related coronaviruses in animals, the economic and political fallout of the SARS epidemic, quarantine law and other public health measures that apply to combating infectious diseases, and the role of international organizations and scientific cooperation in halting the spread of SARS. Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.
