Andrology, a counterpart to gynecology, deals with the study of the male reproductive organs. Clinical andrology has been neglected primarily because of the lack of relevant, accurate laboratory methods for functional analysis, but in the last decade substantial progress has been made in the understanding of male reproductive biology. This progress has resulted from modern tech niques and instrumentation in microanatomy, immunology, neurophysiology, pathology, genetics, endocrinology, biochemistry, biophysics, urology and surgery. These studies are scattered in such a wide spectrum of journals that andrologists can hardly keep abreast of the advances. There have been numerous textbooks on the testes, male accessory organs and semen but the clinical aspects of andrology have not received similar emphasis. Since literature concerning clinical andrology is extensive and widely scattered in many different publica tions, we hope that a useful purpose will be served by summarizing the more pertinent material in a series of volumes which can be made readily available to students of andrology. It was decided that a series of specialized monographs should be devoted to clinics in andrology. These ten volumes of the series are an attempt to coordinate anatomical, physiological, biochemical, endocrinological, pharmacological and immunological aspects of the spermatozoa, testes, epi didymis and other accessory genital organs. Little is known about the effect of diet, diseases, environmental factors and drugs on male reproduction. The interest in developing new male contraceptive methods will stimulate research in andrology.

Intra-uterine insemination (IUI) is a widely used fertility treatment for couples with unexplained and male subfertility. Although it is less invasive and less expensive than l other treatment options such as in vitro fertilization, several factors affect its outcome. In addition, IUI remains controversial due to concerns about some of the possible aspects of treatment, including side-effects such multiple pregnancies. This comprehensive evidence-based book from an international IUI expert team explores all of these topics and provides clear guidelines for daily practice.

Its aim is to increase the number of sperm reaching the fallopian tubes and subsequently increase the chances of fertilisation (American Pregnancy Association). The second edition of Intrauterine Insemination brings physicians and trainees fully up to date with the latest developments in the technique. Divided into 48 chapters, this comprehensive guide covers every aspect of the procedure, from patient selection and clinical assessment of couples, to ovarian induction, predictors of ovarian response, modulation of sperm motility, and sperm banking. Our insight into the mechanisms of the physiology of reproduction has experienced a swift and constant development these last few years. The advent of more sophisticated diagnostic methods and their relatively easy clinical application allow for the incorporation of that knowledge into the evaluation of the infertile couple. These facts, together with an obvious change in social psychology, have facilitated the development of different specialities dealing with the problems of infertility. It is now possible in medical centers all around the world to undertake a better disposition of the infertile husband to look for advice, to be studied, and treated. Confronted with this situation we are still unable to offer specific therapy in most cases; however, empirically based therapy abounds. Advances in therapy have not kept pace with our increased physio logical knowledge and improved diagnostic techniques. Patho physiological mechanisms and etiological factors in male infertility are largely unknown. This has significantly hampered both clinical evaluation and advances in treatment allowing for frequent non scientific therapeutic incursions into the armamentarium of the an drologist. Several factors have given birth to therapeutic "fashions", which are bound to survive as long as this state of lack of knowledge con tinues. For example, we may ask whether the treatment of varicocele constitutes a fashion? Though we accept the treatment of varicocele as the best available treatment of male infertility, we do not know its mechanism of action and so we cannot predict a therapeutic result.

A little over 12 years ago I wrote a small volume entitled Infertility. It seemed to me at that time that significant advances in the field called for the publication of such a volume. The following is from the preface to that volume: During the past 15 years considerable progress has been made in the field of infertility diagnosis and management. It is perhaps a paradox that much of this increased knowledge has come about because of Western medicine's preoccupation with the search for a means to control reproduction. As a result, we have achieved new insights into the physiologic mechanisms involved in reproduction, and we have found better methods for measuring physiologic changes in reproductive health and disease. To these advances can be added improvements in the utilization of endoscopic and surgical techniques, in the diagnosis and treatment of infections and endometriosis, and in the treatment of hormonal disorders. During this period, too, through workshops and conferences and in journals and texts, these latest advances have been made available to physi cians, an outstanding example being the two volumes of Progress in Infer tility, edited by Drs. Jan Behrman and Robert Kistner. As necessary as these publications are, they do not offer an overall view of infertility diag nosis and management.

Even though artificial insemination is a simple technique that has been practiced for over a century, it has long been carried out under poor conditions due to an inadequate understanding of repro ductive physiology and antagonistic socio-ethical attitudes. Accor dingly, until fairly recently it was a medical act with a limited scientific basis which was practised more or less clandestinely. The development of semen preservation has totally changed the conditions of artificial insemination, especially in regard to flexibility and safeguards in its application. Although the use of fresh semen continues, it is now clear that the future of arti ficial insemination is closely linked to semen preservation. During the past two decades, semen banks have been developed in many countries. This has most often been the result of the initiative of individual physicians in either the private or public sectors. In France, a national system of semen banks (CECOS) was begun in 1973. Although there has been cooperation within this system in the areas of both research and management, a need to com municate and compare experiences with those from other countries was perceived. Thus, the first International Symposium on Artifi cial Insemination and Semen Preservation was planned and held in Paris, France in April 1979. Thirty-seven countries were represented byihdividuals from many concerned disciplines.
