This book explains how hypertension affects 20-50% of the adult population in developed countries. Heart failure is the result of the hypertension's effects on the heart and it represents a growing public health problem. In this context the international scientific community is continuously struggling to develop better strategies in screening, diagnosing and treating hypertension and its deleterious effects. Thus, this field is continuously changing, with new important information being added constantly. This volume will offer both insights into the intimate mechanisms of transition from hypertension to heart failure and clinical practice advice on the prevention and treatment of heart failure in hypertensive population. The mechanisms which explain the progression from hypertension to heart failure will be also covered exhaustively by offering two chapters referring to the diagnosis of heart failure in hypertensive population and one regarding echocardiography which is the most frequently used imaging method in clinical practice. The reader will be also provided with information on cardiovascular magnetic resonance which has the unique advantage of differentiating hypertensive heart failure from other pathologies associated with increased myocardial thickness. This book is a useful tool for clinicians but also to the research community interested in heart failure and consequences of hypertension on heart who want to be up-to-date with the new developments in the field.

Arterial hypertension, coronary heart disease and heart fail ure are the commonest cardiovascular conditions to present in clinical practice. Over the past few years it has become in creasingly clear that they are closely and causally interrelated and that their relationship can have a significant bearing on prognosis. Epidemiological studies have shown that arterial hypertension is one of the most important risk factors for de veloping heart failure. Only one in four patients with hyper tension is adequately managed, and in 50% of cases, the hypertension has not been recognised or treated. Patients with pre-existing hypertension who go on to suffer an acute myocardial infarction have usually not previously had typi cal angina symptoms, the infarct territory is larger, life threatening arrhythmias are commoner and hence in-hospi tal mortality and long-term prognosis are markedly worse. The presence of raised blood pressure in the post-infarct phase doubles the risk of manifest heart failure. The close relationship between hypertension, coronary heart disease and heart failure makes the choice of therapeu tic strategy particularly important. Agents and classes of agents that have prognostic value in all three conditions should be considered first, as synergy might result in addi tional benefits. In such patients, this sort of therapeutic deci sion-making might have further advantages. The use of these agents may prevent complications which are not yet clinically obvious (such as heart failure).

Hypertensive heart disease, the number one cause of death associated with hypertension, refers to coronary artery disease, heart failure, and enlargement of the heart that occurs because of high blood pressure. This issue summarizes the current state-of-the-art in diagnosing, treating, and preventing this potentially fatal disease so that cardiologists can offer the best current treatment to their patients.

Hypertensive heart disease is one of the most frequent diseases in man. It is esti mated that there are six to eight million hypertensive patients in the Federal Re public of Germany today. This corresponds with an incidence of 10%-15% in the whole population. Approximately between three and four million show organic eardiac manifestations of hypertension. Of all deaths occurring before the age of 65,30%-40% are attributable to hypertension, and the total morta lity from hypertension is about 20%-25%. Recent epidemiological and clini cal studies have revealed similar figures for other countries, such as the United States and Japan. Hypertensive hypertrophy and cardiac failure are found to be many times more frequent than eardiae hypertrophy and failure of any other origin. Despite the apparent clinical and social importance of the biologieal system "blood pressu re and the heart", data on eardiac performance, on systemic and coronary haemodynamics and on the morphological, bioehemical and contractile proper ti es of ventrieular heart muscle in both experimental and clinical hypertension are rare. This is the reason and stimulus for having organized this symposium, which is the first one coneeming this topie.

Hypertensive Heart Disease written by B.E. Strauer and has been published by Springer Science & Business Media this book supported file pdf, txt, epub, kindle and other format this book has been release on 2012-12-06 with Medical categories.

In the Federal Republic of Germany today there are 6 to 8 million hypertensive patients. Of them, 3 to 4 million show organic cardiac manifestations of hypertension. Of all deaths occurring before the age of 65, 40% are attributable to hypertension, and the total mortality from hypertension is about 25%. The high cardiac morbidity and mortality potential of the risk factor 'hyper tension' lies in the development of cardiac hypertrophy, heart failure and coronary artery disease. In addition, hypertensive patients are predisposed to develop secondary cardiac diseases. The aim of the present study was to analyse the myocardial function and coronary haemodynamics in essential hypertension, i. e. the most common type of pressure load imposed on the human heart. Consequently, the function and mode of operation of the hypertensive heart are described - a type of hypertrophy of the human heart which to date has almost been ignored in pathophysiology and diagnostics. On the basis of the findings, conclusions are drawn for the differential diagnosis and therapy of the hypertensive heart accompanied by hypertrophy, heart failure and coronary artery disease.
