It is an acquired pigmentary disorder afflicting 0.5-2% of the world population for both sexes and all races with a capricious and unpredictable course Even if the precise aetiology and pathobiology of the disease are complex and still debated, recent evidence supports that vitiligo is a T CD8+ cell-mediated autoimmune disease triggered by oxidative stress. The choice of the best therapy for adult and childhood vitiligo is based on various factors enumerated and discussed in the Booklet. Since vitiligo has a deep psychological impact on patients and their quality of life, treating the disease is very important. As dermatologists, they have important goals in the treatment of vitiligo patients: ü stabilization of the disease progression, ü repigmentation of the lesions and ü persistence of the re-pigmentation. Although several medical and surgical therapeutic options have been proposed, no definite cure has yet been developed and the long-term persistence of re-pigmentation is unpredictable. Since the first edition of this book was published, new knowledge has been gained on Vitiligo. This widely revised and updated second edition, written by worldwide experts in the field, fully reflects this progress. The apparently, simple and poorly symptomatic presentation of the disease has been a strong disadvantage to its study, as compared to other common chronic skin disorders such as psoriasis and atopic dermatitis. A good skin-based angle of attack is also lacking because generalized Vitiligo is clearly epitomizing the view of skin diseases as simple targets of a systemic unknown dysregulation (diathesis). This view has mostly restricted Vitiligo to the manifestation of an auto-immune diathesis and skin events, which are easily detected using skin biopsies in most other situations, have not been precisely recorded, with the argument that a clinical diagnosis was sufficient for the management of the patient with Vitiligo.
