Transition planning for young people with special educational needs is a crucial but often overlooked element of social inclusion. While there is now considerable official guidance on how to manage the school leaving process for young people with learning difficulties and/or disabilities, little is known about how to make effective transitions happen in practice. This book supports the transition experiences of young people with a range of special educational needs. The book: Provides insights into the experiences and perspectives of young people, their parents or carers and the professionals who support them during the transition period Explores influences on the decision-making processes and the involvement of young people and their parents or carers Suggests practical ways in which young people and their families and carers can be supported during the transition to adulthood. This is essential reading for Education students, teachers, headteachers, careers guidance and pastoral care personnel, parents or carers and educational psychologists.

Autism, which includes Asperger syndrome, is a lifelong condition which affects the way in which people interact with the world around them. There are estimated to be 400,000 adults with autism in England, many of whom may require specialised support. Yet the NAO found that most NHS organisations and local authorities do not know how many people with autism there are in the areas they serve, and three quarters of local authorities do not have a specific commissioning strategy for adults with autism. GPs and social care staff have low awareness of autism and how to diagnose it, with 80 per cent of GPs surveyed reporting that they need additional guidance and training in order to identify and treat patients with autism more effectively. Around 200,000 adults with autism do not have a learning disability. This group often fails to secure appropriate support, as health and social care services are traditionally configured for people with a learning disability, a physical illness or disability, or a mental health problem (which autism is not). Three quarters of local authorities said adults with autism who do not meet eligibility criteria experience or report difficulties accessing the services they require. Almost two thirds felt that current services for adults with autism are limited. Providing specialised support could improve outcomes for this group of people and their carers, and potentially enhance value for money, as the costs of establishing such support could be outweighed over time by overall savings. There are few specialised employment support services for people with autism. A lack of understanding of autism is a significant barrier to gaining employment and more training is needed for those delivering employment support and those administering benefits.

Successful transition from school to adult life has always been difficult for people with disabilities, especially in the area of employment. The vast majority of people with disabilities are either unemployed or underemployed with low wages and few benefits, and many governments are struggling to find a way of providing employment and benefits to people with disabilities without creating disincentives to work. This book provides strategies and ideas for improving the lives of people with disabilities, exploring new ways of enabling a successful transition to an integrated adult working life by providing effective instruction and support. Following an introduction which outlines the importance of transition services and meaningful outcomes, topics covered in the remaining chapters include: Person Centered Transition Planning; Enhancing Competence and Independence; Employment Assessment and Career Development; Collaboration between Agencies for a Seamless Transition; Independent Living and Supported Living; and Community Functioning Skills. The book will be of interest to all those who work with transition age students as well as those who work with adults with disabilities and want to enable them to have the best life possible. To paraphrase Helen Keller "people with disabilities not only need to be given lives, they need to be given lives worth living."
