There is a growing recognition of the harmful effects of indoor air pollution on health, with recent WHO estimates indicating that indoor smoke from solid fuels causes 1.6 million deaths annually and accounts for 2.7 percent of the global burden of disease. This publication examines the adverse health impacts of indoor air pollution, particularly on children, from fuel use in Guatemala, where the vast majority of poor, rural households use fuelwood as the dominant cooking fuel. It draws on case studies of improved stove programmes to highlight the problem and to explore technical mitigation measures and policy options to improve accessibility to cleaner fuels.

Air pollution in big cities gets headlines, but in many rural areas of developing countries indoor air pollution is an even more serious health problem. Poverty condemns half of humanity to cook with solid fuels on inefficient stoves. As a cause of ill health in the world, indoor air pollution ranks behind only malnutrition, AIDS, tobacco, and poor water/sanitation.Smoke in homes from these cook stoves is the fourth greatest risk factor for death and disease in the world's poorest countries, and is linked to 1.6 million deaths per year. Yet the international community has largely neglected it. This report calls for global action to fight the killer in the kitchen - smoke from cook stoves

Abstract Globally, almost three billion people rely on biomass (wood, charcoal, crop residues, and dung) and coal as their primary source of domestic energy. Exposure to indoor air pollution from the combustion of solid fuels is an important cause of disease and mortality in developing countries. Despite recent advances in estimating the health impacts of indoor smoke, there are limited studies targeted toward the design and implementation of effective intervention programs. We review the current knowledge of the relationship between indoor air pollution and disease, and of the assessment of interventions for reducing exposure and disease. This review takes an environmental health perspective and considers the details of both exposure and health effects that are needed for successful intervention strategies. In particular, we summarize the emerging understanding of the central role of household energy technology and day-to-day household activities in determining exposure to indoor smoke. We also identify knowledge gaps and detailed research questions that are essential in successful design and dissemination of preventive measures and policies. In addition to specific research recommendations based on the weight of recent studies, we conclude that research and development of effective interventions can benefit tremendously from integration of methods and analysis tools from a range of disciplines-from quantitative environmental science and engineering, to toxicology and epidemiology, to the social sciences.

Almost 3 billion people, in low- and middle-income countries mostly, still rely on solid fuels (wood, animal dung, charcoal, crop wastes and coal) burned in inefficient and highly polluting stoves for cooking and heating. In 2012 alone, no fewer than 4.3 million children and adults died prematurely from illnesses caused by such household air pollution, according to estimates by the World Health Organization. Together with widespread use of kerosene stoves, heaters and lamps, these practices also result in many serious injuries and deaths from scalds, burns and poisoning. These new indoor air quality guidelines for household fuel combustion aim to help public health policy-makers, as well as specialists working on energy, environmental and other issues understand best approaches to reducing household air pollution -- the greatest environmental health risk in the world today.
