Cardiovascular diseases have evolved as the main cause of morbidity and mortality worldwide with the frequency expected to increase in the next coming years. Cardiovascular disease summarizes a variety of different pathologies, including but not limited to heart failure, atrial and ventricular arrhythmias, inherited cardiomyopathies, or toxic cardiomyopathy (e.g., alcoholic). Plus, the interaction with important comorbidities, like for example sleep-disordered breathing, further reduces patients’ outcome. Despite recent treatment advances, especially in heart failure patients with reduced ejection fraction, patients’ prognosis remains dramatically reduced, necessitating new therapeutic strategies. This could be achieved by patient-individualized approaches optimized for the various cardiovascular disease entities and their comorbidities. Therefore, detailed understanding of each individual pathomechanism is required.

Chronic pain is an important personal and socio-economic burden, affecting more than 30% of people worldwide. It is highly disabling and associated with long-term affective and cognitive sequelae. Despite synergistic advances in basic and clinical research, some of the most severe conditions remain poorly managed, shifting from nociceptive nature to neuropathic and nociplastic components and major neuropsychiatric complications. Therefore, the understanding of pathophysiological mechanisms and the discovery of new pharmacological targets and innovative approaches are strongly awaited. Important advances in basic research focus on the mechanistic aspect, offering no direct implication for novel therapeutic strategies. Conversely, the use of innovative therapies is not easily considered in clinical practice, breaking the process of translation for therapy. The primary goal of this Research Topic is to create a synergistic circularity of knowledge, merging clinical challenges with the latest pathophysiological findings in the field of pain and related neuropsychiatric disorders, with implications for pharmacological innovation. Cardiovascular diseases are a principal cause of death globally. Cardiovascular diseases include hypertension, atherosclerosis, ischemic heart disease, and heart failure which cause a hike in socioeconomic burden. The disorders can occur due to unhealthy lifestyle like smoking and high-fat diet, genetic, and age. Both experimental and clinical studies are in progress, in search for drugs with improved efficacy and lesser side effects. These studies and experimental research aim to develop novel cardiovascular drugs, many of which are used clinically to alleviate the symptoms and severity of the diseases. For instance, combined therapy of sacubitril/valsartan for patients with heart failure demonstrates better clinical outcomes than the use of valsartan alone. Moreover, recently, clinical indications of originally non-cardiovascular drugs such as sodium-glucose co-transporter-2 inhibitors (antidiabetic drugs) are repurposed for the treatment of cardiovascular diseases. Therefore, it is ultimately eminent for the research community to be updated with recent advances in cardiovascular pharmacotherapy. Reviews in Cardiovascular Pharmacology: 2023 will publish high-quality scholarly review papers on key topics in Cardiovascular Pharmacology. Fibrosis, characterized by uncontrolled extracellular matrix deposition, is a common phenomenon seen in different organs and tissues, bringing about disastrous influence on the function. Fibrosis can be induced by aging and diseases, such as sarcopenia during aging, renal fibrosis following diabetes, liver fibrosis following non-alcohol fatty liver disease; can be elicited by trauma or surgery, such as shoulder stiffness and elbow stiffness; and can also be a feature of the tumor, in which is also known as epithelial-mesenchymal transition. Currently, anti-fibrosis therapies are investigated broadly, while the complexity of this condition retards the development of effect modalities remarkably. Therefore, a better understanding of the mechanisms of fibrosis-related disorders is of urgent need. Recent decades have witnessed remarkable advancement in multi-omic investigations and the progress of pharmacogenomics. Countless potential targets are unveiled by integrating the data of genomics and pharmacology. Since fibrosis and related disorders have various manifestations at the cellular level, proteomic level, and transcriptional level, integrative analysis is of vital importance in identifying targets. Moreover, the interplay between different cells in fibrotic tissue is also finely described by single-cell sequencing and spatial-transcriptome, et al, greatly broadening the understanding of the progress of fibrosis. Therefore, this research topic aims to study the underlying mechanisms of fibrosis and related diseases with specific attention to molecular targets. We welcome research revealing the mechanisms of fibrosis and studies to identify novel molecules against fibrogenesis.

Diabetes, also known as diabetes mellitus (DM), is a disease that occurs when the glucose level in the blood becomes too high (hyperglycaemia). Chronic hyperglycaemia is accompanied by both biochemical and pathological complications such as retinopathy, nephropathy, neuropathy, and cardiovascular diseases. Diabetes mellitus can be classified into two types: type I which results from the destruction of pancreatic β-cells, leading to insulin insufficiency and type II diabetes mellitus, which is mainly related to insulin resistance. Optimal control of blood glucose levels remains the cornerstone of managing DM. To date, the major classes of antidiabetic medications used to treat diabetes include: biguanides, dipeptidyl peptidase 4 (DPP-4) inhibitors, sulfonylureas, meglitinides, thiazolidinediones (TZDs), sodium-glucose cotransporter (SGLT2) inhibitors, α-glucosidase inhibitors, glucose-dependent insulinotropic polypeptide (GIP) receptor and glucagon-like peptide-1 (GLP-1) receptor agonists and the various types of insulin. Several of these drugs may have, besides their antidiabetic properties, also important cardiovascular complications for the patients taking them.
