In 2011, heads of state and government at the United Nations General Assembly formally acknowledged that NCDs are one of the major challenges to development and committed to integrate NCD prevention and control into other programmes, such as for HIV and broader maternal and child health programmes, especially in PHC. The commitment to integrate NCDs into existing disease programmes was reaffirmed again through its NCD resolutions at its Third High-level Meeting in 2018. This guidance outlines strategic actions and practical solutions in response to the challenges of integration of NCDs, as appropriate and relevant to the country context. The objective is to maximize the impact of health services and extend access to NCD care. The target readership is policymakers, programme managers and health providers. The guidance should also be useful for WHO and international partners (including donors and banks) that champion integrated support for NCD service delivery in countries and better management.

Noncommunicable diseases (NCDs) have important implications for pregnancy outcomes and the subsequent health of women and their children. The aim of this study is to determine the status of NCD and maternal health program integration, identify barriers to integration, and explore what would be required to deepen integration of NCD care into antenatal care in Cameroon. We used two methods of data collection and synthesis: a desk review of policy documents and protocols and a series of key informant interviews with health system experts and managers working in public, private, and faith-based health facilities at central, regional, and district levels. Although screening for blood glucose and blood pressure occurs during antenatal care, post-diagnosis management is not well-integrated and often requires referral to specialists in higher-level health facilities. Key barriers to integration include lack of guidelines for the management of NCDs, financial constraints for facilities and patients, and shortages of health workers, medications, and supplies for laboratory investigations. Further integration of services for NCDs during pregnancy will require national guidelines backed up by system-wide strengthening of health information systems, insurance coverage, supply chain management, and human resource capacity, particularly in remote areas.

The COVID-19 pandemic has forced many countries to focus their attention and resources on tackling the disease, paying less attention to other public health issues such as the need to integrate mental health services into HIV care. But “investing in mental health and psychosocial support, and ensuring the integration of mental health and HIV interventions, are critical for achieving universal health coverage, ensuring health equity and ending the AIDS epidemic” (WHO, 2022:6).

On June 11 and June 12, 2019, the National Academies convened a workshop to explore the growing understanding of how the interplay between humans and microbes affects host physiology and causes noncommunicable diseases. Discussions included an overview of colliding epidemics, emerging research on associations between infectious and noncommunicable diseases, risks posed by chronic diseases to the development and severity of infectious diseases, and the influence of the microbiome. Workshop participants also examined the challenges and opportunities of convergence, the integration of health care delivery models and interventions, potential approaches for research, policy, and practice in the immediate-term, and potential directions for the long-term. This publication summarizes the presentations and discussions from the workshop.
