TERMINAL SEDATION DURING THE 1990s During the 1990s a discussion took place in scholarly journals concerning a measure within palliative care that had earlier attracted little attention, to wit, the sedation of dying patients. There seem to have been two main reasons why the practice came under debate. On the one hand, some people felt that, when palliative medicine had advanced and methods to control symptoms had improved, it was no longer justified to sedate the patients in a manner that had often been done in the past. The system of 1 terminal sedation had turned into ‘euthanasia in disguise’ or ‘slow euthanasia’. On the other hand, there were people sympathetic to the recently established Dutch system of euthanasia, people who agreed that terminal sedation was euthanasia in disguise, but who felt that, if it is not objectionable to sedate dying patients at their request, then why should it not be permitted for doctors to kill dying patients at 2 request? From these two motives a discussion about terminal sedation gained momentum. The intention behind this anthology is to continue and deepen this discussion. The anthology starts off with a chapter where an influential article from the 1990s has been reprinted.

Many people might, in fact, agree on the various circumstances in which the death of a person constitutes a violation of the right to life, or that people have a right to our help, especially a right to life-saving aid. In Killing and Saving, Reeder analyzes five sorts of situations in which we are morally permitted or even obligated to take human life: e.g., when we repel an attacker who voluntarily "forfeits" the right to life; when we are confronted with "involuntary pursuit" or "material aggression;" when someone "yields" the right to life; when all will die if nothing is done, but some can be saved if others are killed; and when there is a "double effect" in which we take life as a foreseen but unintended consequence of attempt to achieve a greater good. Reeder argues that these (and closely related) categories account for many of our convictions ranging from abortion to infanticide, to starvation, to war. He also examines the concept of absolute or exceptionless right to life. Reeder draws on a number of moral views, from theological ethics to Enlightenment notions of natural rights or respect for rational creatures. He does not attempt to argue for a foundation for the right not to be killed and the right to be saved. Rather, he focuses on the content of the convictions themselves and argues that where disagreements remain, such as the case of abortion, they can be accounted for by the way the rights in question are explained and justified.

Suicide, and how civilized people should respond to it, is an increasingly controversial topic in modern society. In Holland, suicide is the third leading cause of death of people between the ages of fifteen and forty. In the United States, it is the second leading cause of death among older teenagers. Laws prohibiting assisted suicide are being directly and boldly confronted by activists in the United States, most notably Jack Kevorkian. Meanwhile, the American Civil Liberties Union has publicly declared suicide a fundamental human right that should be protected under the Constitution. The Hemlock Society has introduced referenda in California, Washington, and Oregon to legalize suicide and assisted suicide. The most vocal opposition to these initiatives has come from the Roman Catholic church. "Breaking the Thread of Life "marshalls philosophical, moral, medical, historical, and theological arguments in support of the Roman Catholic position against suicide. In a comprehensive study of the history of suicide, Barry shows that Christian civilization was one of only a few early societies that was able to bring suicide under control. He counters claims that Catholicism and the Bible endorse rational suicide. Barry presents the rationale for the Roman Catholic church's strong, extensive, and articulate opposition to efforts to gain legal and social endorsement of suicide and assisted suicide.
