The clinical significance of tumor spread has always been appreciated. Yet, in spite of the pioneering work and outstanding contributions of investigators such as D. Coman, H. Green, B. Fisher, S. Wood and I. Zeidman, studies on metastasis rarely achieved the popularity afforded to more esoteric areas of tumor biology. Tumor dissemination, occurring as it does in a responding host and being composed of a series of dynamic int~ractions, is a highly complex phenomenon. Few investigators were brave enough to attempt to unravel the mechanisms involved. Paradoxically, this very complexity may have contributed, in part, to the recent upsurge of interest in metastasis research. More and more researchers are becoming fascinated by the complexities of the cellular interactions involved in tumor spread. Accompanying this intellectual stimulation have been technological advances in related fields which allow the derivation of new model systems. The mechanisms of metastatic spread are increasingly amenable to both the reductionist and holistic approaches and it is the purpose of this volume to present many of these model systems while emphasizing the intricacy and complexity of the processes they mimic. We have attempted to emphasize two topics not previously covered in depth in previous books on metastases. These are in vitro models of invasion and in teractions of tumor cells with connective tissue.

Metastasis continues to be the leading cause of breast cancer-related deaths. Metastasis is a multistep process during which cancer cells leave the primary tumor, invade and seed inside secondary tissues, leading to lethal organ failure. Currently, no efficient method exists for the elimination of invasive cancer cells, which poses a major clinical challenge. Indeed, while localized cancers are clinically manageable, invasive breast cancer is largely an uncurable disease. Understanding how cancer cells sustain an invasive behavior may lead to the development of novel potent therapies.A vast genetic and phenotypic heterogeneity exists among invasive cancer cells. This heterogeneity is important to investigate as, in breast cancer patients, most metastatic nodules are polyclonal and consist of cancer cells that invaded and disseminated collectively. While recent studies demonstrated that collective invasion can be cooperative, where leader cells enable the invasion of otherwise non-invasive follower cells, the mechanisms regulating their cooperativity have been poorly investigated. Additionally, it is not known whether cooperative invasion can lead to metastasis, where metastatic cells enable the dissemination of non-metastatic cells. Our laboratory previously showed that breast cancer cells can locally degrade the extracellular matrix using invadopodia, which are membrane protrusions enriched in matrix metalloproteinases. Importantly, invadopodia are required for metastasis of single cancer cells. However, whether invadopodia enable the collective invasion and metastasis of cancer cell clusters remains unknown. Further, the role of invadopodia-mediated matrix degradation in leader vs. follower cells is unclear. In this dissertation, we aim to understand how cancer clones with and without invadopodia may cooperate during invasion and metastasis. Our results suggest that a few cells with invadopodia can drive the metastasis of cell clusters from multiclonal tumors. From this work, we believe that a more detailed understanding of how cancer cells cooperate may help in identifying new targets for preventing metastasis.

The past twenty years have witnessed significant advances in the treatment of cancer by surgery and radiation therapy. Gains with cytotoxic chemotherapy have been much more modest. Of the approximately 900,000 newly diagnosed cases of cancer each year, 50010 result in death of the patient. The primary cause of these deaths is metastasis. Although the term metastasis was first coined by Recamier in 1829, only in the past ten years have there been intensive scientific investigations into the mechanisms by which tumor cells metastasize. What has emerged is a complex process of host-tumor cell interactions which has been termed the metastatic cascade. Due to the complexity of the metastatic process, the study of metastasis is multifaceted and involves elements of such areas as differentiation, en zymology, genetics, hematology, immunology, membrane biochemistry and molecular biology. The major objectives of this book were to present the most recent advances in our understanding of how tumor cells metastasize to secondary sites by the leading experts in the biology of tumor invasion and metastasis. We hope that this book will lead to new concepts for the treatment of subclinical metastatic cancer. The chapters in this book address both the basic science of metastasis and potential clinical therapies directed toward interruption of the metastatic cascade or toward eradication of subclinical metastases. Many relevant topics have been omitted due to space considerations and thus the topics included reflect the prej udices of the editors.
