Investing in Communities Achieves Results fills an important gap in the global knowledge on community level results and resources related to HIV and AIDS. While communities, in spite of their limited resources, have played a key role in the HIV/AIDS response, their contributions and innovative approaches to prevention, treatment, care, and support have not always been the focus of systematic and rigorous evaluations. To address this deficit, a series of studies-including evaluations in Burkina Faso, India, Kenya, Lesotho, Nigeria, Senegal, South Africa, and Zimbabwe-were undertaken over a three-year period (early 2009 to early 2012), helping to build a robust pool of evidence on the effects of community-based activities and programs. A unique feature of this multicountry evaluation was the collaboration between two international organizations (the World Bank and the United Kingdom's Department for International Development) and a major civil society network (the U.K. Consortium on AIDS and International Development). Other attributes that contributed to the successful outcome were the sustained consultation process with civil society and stakeholders at the local, national, and global levels, and the collaboration among high-caliber, multi-disciplinary researcher teams. The book's findings are promising. At varying levels, depending on the country context, the HIV response in communities was shown to improve knowledge and behavior and increase the use of health services- and even decrease HIV incidence. Evidence on social transformation was more mixed, with community groups found to be effective only in some settings. Each study in the evaluation provides a partial view of how communities shape the local response; however, taken together they constitute a significant pool of rigorous evidence on the contributions of communities, community groups, and civil society to the national and global HIV and AIDS response. The studies suggest that communities have produced significant results at the local level, which contribute to outcomes at the national level.

In August 2008, Heads of State of the Southern African Development Community adopted the ground-breaking SADC Protocol on Gender and Development. This followed a concerted campaign by NGOs under the umbrella of the Southern Africa Gender Protocol Alliance. The SADC Gender Protocol is the only sub-regional instrument that brings together existing global and continental commitments to gender equality and enhances these through time bound targets. Aligned to Millennium Development Goal Three, the original 28 targets of the Protocol targets expire in 2015. Now that 2015 is here, we need to step back, assess and reposition. In June 2014, SADC Gender Ministers agreed to review the targets of the Gender Protocol in line with the Sustainable Development Goals (SDGs). In May this year, ministers added that they want the Protocol to be accompanied by a Monitoring, Evaluation and Results Framework. The 2015 Barometer shows that implementation is now the biggest missing gap in the quest for gender equality. The basic premise of this book is that the conversation on the future of development needs to shift from a focus on poverty to that of inequality. The poverty emphasis is in an intellectual and political cul de sac. It does not address the fundamental question of why people are poor nor what can be done structurally and institutionally to reduce and eliminate it. The various chapters illustrate in the context of various countries and sectors around the world, the significant contributions that evaluators can make in terms of improvement of the analytical framework, analysis of the performance and results of specific programs and projects, as well as assessing and designing better public management systems in terms of poverty and inequality reduction. Beyond the specific contributions presented, three characteristics characterize those evaluations to be relevant for poverty and inequality analysis: a global-local approach: Global to move beyond disciplinary boundaries and consider cross-cutting issues, local to account for the diversity of countries, sectors, institutions and cultures considered; a problem-solving orientation: The issue evaluated is the core focus and determines the choice of evaluation methods to analyze this issue from a variety of angles; an evolutionary approach: Chapters presented are from iconoclasts who do not have any pre-established theory or school of thought to defend. This is the result of openness of mind and ability to adapt the analytical framework, the evaluation methods, and the interpretation of results in a constant interaction with the stakeholders. Such characteristics make evaluation a domain that can help understand better complex issues like poverty, inequality, vulnerability, and their interactions as well as propose a relevant and useful theory of change for public policies and projects to improve the plight of a large part of the world population in industrialized and developing countries alike.

In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways.
