The Robert Wood Johnson Foundation asked the Institute of Medicine (IOM) to examine three topics in relation to public health: measurement, the law, and funding. IOM prepared a three report series-one report on each topic-that contains actionable recommendations for public health agencies and other stakeholders with roles in the health of the U.S. population. For the Public's Health: Investing in a Healthier Future, the final book inthe series, assesses the financial challenges facing the governmental public health infrastructure. The book provides recommendations about what is needed for stable and sustainable funding, and for its optimal use by public health agencies. Building on the other two volumes in the series, this book makes the argument that adequate and sustainable funding for public health is necessary to enable public health departments across the country to inform and mobilize action on the determinants of health, to play other key roles in protecting and promoting health, and to prepare for a range of potential threats to population health. This Review sets out to propose a structure for the funding arrangements for the whole spectrum of health research, with the objective of obtaining the maximum benefit from research success and, where possible, eliminating duplication of effort. The Review found, however, that the UK is at risk of failing to reap the full economic, health and social benefits that the UK's public investment in health research should generate. There is no overarching UK health research strategy to ensure UK health priorities are considered through all types of research and there are two key gaps in the translation of health research: (i) translating ideas from basic and clinical research into the development of new products and approaches to treatment of disease and illness; (ii) implementing those new products and approaches into clinical practice.The Review also found that the wider funding arrangements for supporting translation of ideas from conception to practice could be more coherent or comprehensive and, where arrangements exist, they do not function well. The Review identified cultural, institutional and financial barriers to translating research into practice in the publicly funded research arena. But it also found that, in the private sector, the pharmaceuticals industry is facing increasing challenges in translating research into health and economic benefit. The Review has sought to make recommendations that will increase the translation of R&D into health and economic benefit for the UK, both in the public and private sectors. The Review recommends that the Government should seek to achieve better coordination of health research and more coherent funding arrangements to support translation by establishing an Office for Strategic Coordination of Health Research (OSCHR).

Young adulthood - ages approximately 18 to 26 - is a critical period of development with long-lasting implications for a person's economic security, health and well-being. Young adults are key contributors to the nation's workforce and military services and, since many are parents, to the healthy development of the next generation. Although 'millennials' have received attention in the popular media in recent years, young adults are too rarely treated as a distinct population in policy, programs, and research. Instead, they are often grouped with adolescents or, more often, with all adults. Currently, the nation is experiencing economic restructuring, widening inequality, a rapidly rising ratio of older adults, and an increasingly diverse population. The possible transformative effects of these features make focus on young adults especially important. A systematic approach to understanding and responding to the unique circumstances and needs of today's young adults can help to pave the way to a more productive and equitable tomorrow for young adults in particular and our society at large. Investing in The Health and Well-Being of Young Adults describes what is meant by the term young adulthood, who young adults are, what they are doing, and what they need. This study recommends actions that nonprofit programs and federal, state, and local agencies can take to help young adults make a successful transition from adolescence to adulthood. According to this report, young adults should be considered as a separate group from adolescents and older adults. Investing in The Health and Well-Being of Young Adults makes the case that increased efforts to improve high school and college graduate rates and education and workforce development systems that are more closely tied to high-demand economic sectors will help this age group achieve greater opportunity and success. The report also discusses the health status of young adults and makes recommendations to develop evidence-based practices for young adults for medical and behavioral health, including preventions. What happens during the young adult years has profound implications for the rest of the life course, and the stability and progress of society at large depends on how any cohort of young adults fares as a whole. The enactment of the America COMPETES Act in 2006 (and its reauthorization in 2010), the increase in research expenditures under the 2009 American Recovery and Reinvestment Act (ARRA), and President Obama's general emphasis on the contribution of science and technology to economic growth have all heightened interest in the role of scientific and engineering research in creating jobs, generating innovative technologies, spawning new industries, improving health, and producing other economic and societal benefits. Along with this interest has come a renewed emphasis on a question that has been asked for decades: Can the impacts and practical benefits of research to society be measured either quantitatively or qualitatively? On April 18-19, 2011, the Board on Science, Technology, and Economic Policy (STEP) and the Committee on Science, Engineering and Public Policy (COSEPUP) of the National Academy of Sciences, the National Academy of Engineering, and the Institute of Medicine, held a workshop to examine this question. The workshop sought to assemble the range of work that has been done in measuring research outcomes and to provide a forum to discuss its method. The workshop was motivated by a 2009 letter from Congressman Rush Holt (D-New Jersey). He asked the National Academies to look into a variety of complex and interconnected issues, such as the short-term and long-term economic and non-economic impact of federal research funding, factors that determine whether federally funded research discoveries result in economic benefits, and quantification of the impacts of research on national security, the environment, health, education, public welfare, and decision making. Measuring the Impacts of Federal Investments in Research provides the key observations and suggestions made by the speakers at the workshop and during the discussions that followed the formal presentations.

In its 2009 report, Beyond the HIPAA Privacy Rule: Enhancing Privacy, Improving Health Through Research, the Institute of Medicine's Committee on Health Research and the Privacy of Health Information concludes that the HIPAA Privacy Rule does not protect privacy as well as it should, and that it impedes important health research.
