The tenth Henry Goldberg Workshop is an excellent occasion to recall our goals and celebrate some of our humble achievements. The first Goldberg Workshop, held in Haifa, in 1984, explored the interaction of mechanics, electrical activation, perfusion and metabolism, emphasizing imaging in the clinical environment. The second Workshop, in 1985, discussed the same parameters with a slant towards the control aspects. The third Goldberg Workshop, held in the USA at Rutgers University, in 1986, highlighted the transformation of the microscale activation phenomena to macro scale activity and performance, relating electrophysiology, energy metabolism and cardiac mechanics. The fourth Goldberg Workshop continued the effort to elucidate the various parameters affecting cardiac performance, with emphasis on the ischemic heart. The fifth Workshop concentrated on the effect of the inhomogeneity of the cardiac muscle on its performance. The sixth Workshop highlighted new imaging techniques which allow insight into the local and global cardiac performance.

Living organisms exhibit specific responses when confronted with sudden changes in their environmental conditions. The ability of the cells to acclimate to their new environment is the integral driving force for adaptive modification of the cells. Such adaptation involves a number of cellular and biochemical alteration including metabolic homeostasis and reprogramming of gene expression. Changes in metabolic pathways are generally short-lived and reversible, while the consequences of gene expression are a long-term process and may lead to permanent alternation in the pattern of adaptive responses. The heart possesses remarkable ability to adapt itself against any stressful situation by increasing resistance to the adverse consequences. Stress composes the foundation of many degenerative heart diseases including atherosclerosis, spasm, thrombosis, cardiomyopathy, and congestive heart failure. Based on the concept that excessive stress may play a crucial role in the pathogenesis of ischemic heart disease, attempts were made to design methods for preventing of myocardial injury. Creation of stress reactions by repeated ischemia and reperfusion or subjecting the hearts to heat or oxidative stress enables them to meet the future stress challenge. Repeated stress exposures adapt the heart to withstand more severe stress reactions probably by upregulating the cellular defense and direct accumulation of intracellular mediators, which presumably constitute the material basis of increased adaptation to stress. Thus, the powerful cardioprotective effect of adaptation is likely to originate at the cellular and molecular levels that compose fundamental processes in the prophylaxis of such diseases. The immense growth in the number of articles on adenosine cardioprotection in recent years has been related in large part to the hypothesis that adenosine plays a role in ischemic preconditioning. Ischemic preconditioning is the phenomenon in which a brief period of ischemia (and reperfusion) prior to a more prolonged occlusion reduces myocardial infarct size. This form of myocardial protection has received much interest because ischemic preconditioning has been shown to be the most potent means of reducing infarct size in all animal models so far tested. Within the last six years, the principal focus of adenosine cardioprotection research has centered on the role of adenosine receptors. It is currently thought that adenosine protects the ischemic heart primarily via the activation of adenosine A1 receptors located on the cardiac myocytes. Adenosine is used clinically for terminating supraventricular tachycardia, and as a diagnostic tool in coronary imaging, and has been used postoperatively for blood pressure control after heart surgery. There are also recent reports that adenosine may be safely tolerated and a potentially beneficial additive to cardioplegic solutions during open heart surgery in humans. There is even evidence that ischemic preconditioning may occur in humans under various clinical situations.
