Excerpt from Journal of Orificial Surgery, Vol. 6: 1897-98 Examining more closely, now, the vaginal walls, we will do what we can with scissors and curette in the way of smoothing the sur face, after which, by the aid of the rectal bivalve, we will give the vaginal walls all the tension they will bear without causing a rent. Having accomplished this, let us give attention to the parts about the pudenda. First of all, with the spud end of the hysterectomy knife we will loosen the hood of the clitoris and clear away the smegma, smearing the parts with lanoline. We will now clip away the roughened and hardened projections about the orifices of the urethra and vulva, after which we will amputate the thickened and hardened margins of the labia minora. There is little bleeding, as the parts are so lifeless. By a continuous thread of fine catgut we will close the margins of the wound made by the removal of the labia minora. At first sight it may appear strange that a volume of Progress in Pediatric Surgery should be devoted to the history of our specialty. One assumes that progress is concerned primarily with recent developments whilst history deals with matters of the past. However, in the past there has also been considerable progress in the development of our understand ing of paediatric surgical problems, otherwise we would not have pro gressed to our present achievements. The editors, therefore, do not apologize for compiling this volume but, on the contrary, feel that the publication of this volume is most timely. Modern paediatric surgery has now been practised for three genera tions. The handful of pioneers who were the founders of our specialty worked mainly before the last world war. A few dozen of the inter mediary generation started work immediately after the war, while the new generation who are now dominating our specialty must be counted in thousands. Two factors have radically altered paediatric surgery as practised by the intermediary and the present generation of surgeons.

Minimally invasive surgery has become a common term in visceral as well as gynecologic surgery. It has almost evolved into its own surgical speciality over the past 20 years.
