Excerpt from Journal of Orificial Surgery, Vol. 6: 1897-98 Examining more closely, now, the vaginal walls, we will do what we can with scissors and curette in the way of smoothing the sur face, after which, by the aid of the rectal bivalve, we will give the vaginal walls all the tension they will bear without causing a rent. Having accomplished this, let us give attention to the parts about the pudenda. First of all, with the spud end of the hysterectomy knife we will loosen the hood of the clitoris and clear away the smegma, smearing the parts with lanoline. We will now clip away the roughened and hardened projections about the orifices of the urethra and vulva, after which we will amputate the thickened and hardened margins of the labia minora. There is little bleeding, as the parts are so lifeless. By a continuous thread of fine catgut we will close the margins of the wound made by the removal of the labia minora. A promising long-term evolution of surgery relies on intracorporeal microrobotics. This book reviews the physical and methodological principles, and the scientific challenges to be tackled to design and control such robots. Three orders of magnitude will be considered, justified by the class of problems encountered and solutions implemented to manipulate objects and reach targets within the body: millimetric, sub-millimetric in the 10- 100 micrometer range, then in the 1-10 micrometer range. The most prominent devices and prototypes of the state of the art will be described to illustrate the benefit that can be expected for surgeons and patients. Future developments nanorobotics will also be discussed.

First Conquer Thyself: Socialization of violence and abuse is often highly structured through the Munchausen Complex. Munchausen Syndrome occurs when an individual harms themselves for attention and self-glorification. Munchausen Syndrome by Proxy is when an individual harms another, usually under their care. Attention and self-glorification are achieved through their victim's subsequent medical treatment. Munchausen Syndrome by Proxy is a crime with a victim. Violence and abuse are often common in families, passed down from one generation to the next and may be termed Transgenerational Munchausen Syndrome as these families expand and their habits are introduced into society. Munchausen Syndrome in Collective Transmission occurs when such practices become an acceptable part of society often eventuating into full acculturation. In this way societies attach themselves to self-glorification with various explanations of justification. A Mandated Report is required to be filed by health care, legal, social service and educational professionals who suspect child abuse, as well as developers of film. As socialized beings, we are often unaware why we think what we think and why we do what we do. Exploring beneath the surface, we may discover we are not who we imagine we are. Are our subtle perversions and aberrations so different from those of the ancients, or other cultures we label primitive? The author was under military orders not to discuss the subjects presented in this part's Mandated Report on child abuse to the social body for the last six years of his U.S. Navy career. Censorship is strong when one questions social norms, folkways and rituals that address issues of identity. Internal, interpersonal and professional conflicts arise.

Are you interested in using NOTES to treat your patients? Do you need a multimedia tool to guide you through all aspects of clinical management? Just as laparoscopic surgery revolutionized surgical practice in the 1980’s and 90’s, offering genuine competition to traditional open surgery, Natural Orifice Translumenal Endoscopic Surgery (NOTES) presents a genuinely different alternative for surgeons and patients alike as we move forward in the 21st century. Advantages over laparoscopic surgery include lower anesthesia requirements, faster recovery and shorter hospital stays, avoidance of transabdominal wound infections, less immunosuppression, better postoperative pulmonary and diaphragmatic function and of course, the potential for "scarless" abdominal surgery. Combining high-level text with a superb companion website, Natural Orifice Translumenal Endoscopic Surgery (NOTES): Textbook and Video-Atlas provides you with a hands-on, in-depth and practical multimedia tool on this exciting breakthrough, to consult either on the ward or in surgery. Highlights include: Full coverage of the fundamentals of NOTES, its current clinical applications and techniques, and future perspectives Over 50 outstanding videos illustrating NOTES being performed by the experts Full-color throughout and superbly illustrated with over 250 figures Edited by global leaders and pioneers in the field, who have recruited a world-class contributor team As well being a valuable text for gastroenterologists, GI surgeons and general surgeons, Natural Orifice Translumenal Endoscopic Surgery (NOTES): Textbook and Video-Atlas will also appeal to urologists, urologic surgeons, gynecologists, gynecologic surgeons, thoracic and ENT surgeons.
