Il presente lavoro ha lo scopo di esaminare e approfondire il rapporto tra il corpo femminile, la malattia (isteria e anoressia) e le cause scatenanti la malattia. Nel Medioevo uomini e donne vivevano il digiuno come uno strumento di purificazione dalle tentazioni e dai peccati mondani, al punto che si parlò di santa anoressia o di digiuni ascetici, capaci di elevare lo spirito al di sopra dei bisogni della carne. L’anoressia è il sintomo di un forte disagio ed è stata oggetto di varie interpretazioni: attraverso il dominio esercitato sul corpo, l’anoressica crede di trarre la sua forma di benessere, ma soprattutto la titolarità assoluta della propria vita. Questo meccanismo scatta in Fosca, creatura mortifera e dalla sessualità oscura, protagonista dell’omonimo romanzo di uno degli autori simbolo della Scapigliatura italiana, Igino Ugo Tarchetti. Se da un lato la nevrosi diventa la metafora usata da Tarchetti per descrivere l’ideale patologico dell’amore in cui il carnefice è donna, dall’altro, l’anoressia manifesta il disagio di tante altre donne incastrate in rapporti d’amore sbagliati, permeati dal possesso, dall’incapacità di svincolarsi dall’autorità dell’Altro.

Il secondo volume della collana Behavioral Neurochemistry "Disturbi dell'alimentazione: il punto di vista biologico" di D. Gigli, V. Zannoni, C. Rocca affronta i disordini alimentari da una prospettiva prettamente fisiologica. La collana Behavioral Neurochemistry nasce dal lavoro di ricerca del Gruppo di Neurochimica del Comportamento diretto dal Prof. Gigli all'Università E-Campus di Novedrate. Nel testo vengono presentati i meccanismi biologici alla base della funzione alimentare e le loro implicazioni nello sviluppo e nel mantenimento dei disturbi alimentari. Viene presentato l'insieme dei processi nervosi ed endocrini che regolano l'assunzione di cibo, le strutture anatomiche coinvolte e le loro principali modificazioni nei soggetti affetti da disturbi dell'alimentazione. Infine viene presentata una piccola rassegna dei principali studi di genetica del comportamento per presentare gli sviluppi futuri degli studi neurobiologici. In particolare si prendono in esame le vie dopaminergiche il cui compito è quello di intervenire nella scelta dei cibi e nelle preferenze alimentari. L’attivazione dopaminergica, a livello mesolimbico (amigdala-accumbens), comporta la comparsa di aspettative di gratificazione. Ciò porta alla memorizzazione dell’effetto gratificante nell’ippocampo, al suo apprendimento e quindi alla sua ripetitività. Dunque il sistema mesolimbico, in particolare l’amigdala, l’ippocampo e la parte ventrale del putamen, cioè il nucleus accumbens, costituisce l’apparato preposto alla percezione della gratificazione: una “cascata della gratificazione” che interessa prima il rilascio della serotonina, in grado a sua volta di stimolare le encefaline a livello ipotalamico. A sua volta a livello della sostanza nigra l’inibizione prodotta dalle encefaline sul GABA promuove il rilascio di dopamina nel nucleus accumbens. Ecco perché l’accumbens viene chiamato “centro del piacere” e la dopamina “molecola del piacere” o “molecola antistress”. Dunque la dopamina ha un ruolo fondamentale nella motivazione e nella ricompensa, dato che aumenta sia prima che durante un’attività piacevole (il cibo, il sesso), stimolando le persone a ripetere quell’attività, fondamentale per la sopravvivenza della specie. I percorsi dopaminergici, inoltre, collegano il sistema limbico, che si occupa delle emozioni, con l’ippocampo, che invece è responsabile della memoria. In questo modo, le attività piacevoli vengono collegate a ricordi intensi ed allettanti. Quando il ricordo e il desiderio di ripetere l’attività piacevole prendono il sopravvento, la dopamina aumenta eccessivamente, rompendo i freni inibitori: in chi soffre di dipendenza si nota un’ipofunzione dei lobi frontali, responsabili del controllo e della forza di volontà. Una contrapposizione tra i meccanismi del piacere, che risultano manomessi, e i lobi frontali, che non riescono a impedire l’impulso a ripetere l’esperienza è, dunque, l’essenza della dipendenza da una sostanza o abitudine. Una moderna visione d’insieme mette in relazione i disturbi del comportamento alimentare a tutte le dipendenze, sia da sostanze tossiche (droghe, alcol, fumo) ma anche da gioco d’azzardo, da sesso, da shopping. Dunque l’anoressia può essere compresa nell’ambito delle sindromi di addiction e ciò può essere uno stimolo a nuove ricerche per meglio capire e prevenire la vulnerabilità all’anoressia stessa, come agli altri disturbi del comportamento alimentare.
