Ce roman succède aux Nourritures terrestres et, à bien des égards, il illustre, par le biais de la fiction, les principes de l'ouvrage précédent. Le personnage de Ménalque est d'ailleurs présent dans les deux oeuvres et y joue le même rôle d'initiateur. L'Immoraliste, écrit après l'expérience libératrice vécue en Afrique du Nord dès 1893 et après le mariage de l'auteur, comporte à l'évidence des éléments autobiographiques. Gide s'est toutefois toujours vivement défendu d'être lui-même Michel, son héros. Les amis de Michel, venus, sur son appel, le rejoindre en Afrique du Nord, écoutent sa confession. Michel, jeune homme éduqué selon les valeurs puritaines, est promis à un brillant avenir scientifique, lorsqu'il part en voyage de noces avec sa femme Marceline qu'il a épousée sans véritable amour, pour rassurer son père mourant. La tuberculose, dont Michel se découvre atteint en Tunisie, va bouleverser la destinée de ces deux êtres. Ils s'arrêtent à Biskra où, à force de soins et d'amour, Marceline parvient à sauver son mari. Convalescent, celui-ci préfère à la compagnie de sa femme celle des enfants arabes. La maladie a laissé à Michel un appétit de vivre et une avidité de sensations qu'il avait jusqu'alors ignorés...
