Questo volume è dedicato all’approfondimento dei più accreditati interventi strutturati per la riabilitazione cognitiva dei soggetti affetti da schizofrenia, e rappresenta uno strumento di lavoro per tutti i professionisti impegnati nel trattamento, nella riabilitazione e nel recupero delle persone con disturbi psicotici, in particolare di tipo schizofrenico. La presenza di alterazioni cognitive nei disturbi schizofrenici è attualmente oggetto di grande interesse da parte di clinici e ricercatori, sia per la diffusione di tali alterazioni sia per il loro impatto negativo sulla vita sociale, relazionale e lavorativa del paziente; determinante è stato inoltre il recente sviluppo di nuove tecniche e strategie non farmacologiche di intervento, che costituiscono nel loro complesso la cosiddetta riabilitazione cognitiva della schizofrenia. Dopo i primi capitoli dedicati all’analisi aggiornata delle principali alterazioni cognitive presenti nella schizofrenia e del loro impatto sul funzionamento sociale, vengono passati in rassegna gli strumenti di assessment dei deficit cognitivi e psicosociali, utili per disegnare i successivi interventi e monitorarne gli effetti. Alcuni capitoli affrontano poi il background teorico e i principi applicativi alla base del rimedio cognitivo nella schizofrenia e delle principali tecniche finora sviluppate. In particolare ne vengono analizzate tre, fornite di ampie dimostrazioni di efficacia: la Terapia Psicologica Integrata (IPT) secondo Brenner e colleghi, la Terapia di Rimedio Cognitivo (CRT) secondo Wykes e colleghi, e il Cogpack, rimedio cognitivo svolto con l’ausilio del computer. Si affronta infine il problema dell’implementazione pratica delle tecniche descritte nei servizi psichiatrici territoriali e ospedalieri, suggerendone modalità di integrazione con gli interventi terapeutici e riabilitativi abituali.

The book brings together into a single text the interrelated but different research efforts to translate the current evidence on risk and outcome of severe mental disorders into a preventive perspective. The book also introduces a holistic approach to prevention in mental health, by combining biological, psychological and environmental evidence that attempts to blunt the risk and reduce the number of individuals with mental health vulnerabilities who eventually progress to the manifestation of a severe mental disorder. Finally, the book wants also to highlight the possibility to overcome the single disorder-oriented preventive approach in an attempt to intercept a wider at-risk youth population and explore clinical research areas underperformed where future efforts will have to concentrate. Mental health problems have their peak of incidence during the transition from childhood to young adulthood, interesting up to 20% adolescents. Half of those eventually developing such difficulties experience clinically relevant mental distress by the age of 14. Even more importantly, the symptomatic onset is generally anticipated by non-specific warning signs of psychosocial impairment potentially evolving in any severe mental disorder. This is of crucial importance, as almost one in two health problems contributing to the global disease burden across the 0-25 age span is a mental disorder. The search for preventive strategies among youth has developed over the past 2-3 decades, invigorated by a rethinking of mental disorders’ ineluctable prodromal phase into a period where the trajectory of illness can be slowed down, blunted, or even halted. The paradigms for implementing preventing approaches in mental health have often developed independent of each other. This book aims at summarizing the available evidence and make a step towards a more mature vision of the potentialities of promotion and prevention in mental health.

In the roughly two decades since Aaron T. Beck published the now classic "Cognitive Therapy of Depression," and Michael J. Mahoney declared the "Cognitive Revolution," much has happened. What was proposed as the "cognitive revolution" has now become the zeitgeist, and Cognitive Therapy (CT) has grown exponentially with each passing year. A treatment model that was once seen as diffe rent, strange, or even alien, is now commonplace. In fact, many people have allied themselves with CT claiming that they have always done CT. Even my psychoanalytic colleagues have claimed that they often use CT. "After all," they say, "Psychoanalysis is a cognitive therapy." Cognitive Therapy (or Cognitive Psychotherapy) has become a kaleidoscope model of treatment, with influences coming from many sources. Some of these contributory streams have been information pro cessing, behavior therapy, Constructivist psychology, and dynamic psychotherapy. Each of these sources have added color, shading, and depth to the CT model. What was originally uni dimensional in terms of the CT focus on depression has become multidimensional as the CT model has been applied to virtually every patient population, treatment setting, and therapy context. CT must now be seen as a general model of psychotherapy that, with modifications, can be applied to the broad range of clinical problems and syndromes. What has tied these various applications of CT together is the emphasis on a strong grounding in cogni tive theory, a commitment to empirical support, and a dedication to broadening the model.
