El trastorno límite de la personalidad es una condición psicopatológica bien caracterizada, conocida y formalizada desde hace más de 80 años. Se trata de un trastorno con diferentes niveles de gravedad, sin embargo en la práctica clínica se presentan los pacientes más severos, los cuales por lo general presentan intentos de suicidio recurrentes algunos de ellos consumados, múltiples conductas impulsivas, relaciones interpersonales inestables y caóticas, depresión, inestabilidad afectiva y frecuente coocurrencia con otros trastornos psiquiátricos. Los pacientes TLP suelen padecer también múltiples enfermedades médicas relacionadas con las dificultades de cuidar la propia salud. Todos estos factores contribuyen al alto grado de utilización de servicios médicos y gastos cuantiosos en salud pública. Este volumen tiene la modesta finalidad de presentar un modelo teórico-clínico que considera al trastorno límite como el resultado final y dinámico de la interacción de múltiples niveles de análisis desde el genético-molecular al nivel psicosocial (interacción gen-ambiente). El objetivo principal es comenzar a ayudar al lector a considerar una visión contemporánea de esta severa patología en concordancia con las investigaciones contemporáneas del diagnóstico en psiquiatría basadas en parte en el modelo del proyecto "Research domain criteria" (RDoC) propuesto por el instituto de salud de los Estados Unidos (NIMH) que intenta investigar endofenotipos y procesos neurobiológicos que faciliten el diagnóstico y el tratamiento. La psicopatología TLP es abordada desde el nivel genético, la neuroanatomía funcional y la posible respuesta farmacológica. Se aborda un paradigma muy actual en la consideración de la patología del trastorno límite: el procesamiento mental basado en el paradigma de la teoría de la mente como una forma profunda de considerar los procesos de autoconciencia, capacidad de reflexión y empatía, algunas de estas capacidades profundamente disfuncionales en algunos pacientes borderline.
