Abstract: Purpose. To determine if canine lens epithelial cells (LEC) contain the hyaluronic acid (HA) receptors CD44 and RHAMM, and if HA can impact posterior capsular opacification (PCO) formation in vitro. Methods. Normal (n=10) and cataractous (n=10) canine LEC were evaluated using immunohistochemistry and RT-PCR for expression of CD44 and RHAMM. In vitro models of PCO formation were used to determine if various HA concentrations altered LEC migration. A one-millimeter scratch was created in cultures of confluent canine LEC. Cells were treated with 0, 0.02, 0.012, or 1.0 mg/mL (n=6 per group) of HA dissolved in culture media. Migration of LEC into the scratch was quantified. Mock cataract surgery was performed on canine cadaver eyes. Capsules were re-distended using one of the following (n=6 per group): PBS only, hydroxypropyl methylcellulose viscoelastic, 1.2% HA (12 mg/ml HA) viscoelastic, or 2.0% (20 mg/ml HA) HA viscoelastic. This book is the first to summarize the current knowledge of the cell biology of lens epithelial cells in relation to and in the development of posterior capsular opacification (PCO). PCO remains the most common long-term complication of modern cataract surgery, occurring months or years after cataract surgery, unlike most other complications that tend to arise during or soon after the procedure. Opacification of the posterior capsule appears to be linked to lens epithelial cells that are left behind in the eye during cataract removal. These cells proliferate, migrate across the posterior lens capsule, and undergo changes that result in fibrous or pearl-type opacities in the capsule. The first section of the text explains the molecular mechanism and biology of lens epithelial cells that lead to the incidence of PCO. In the second part, in addition to a description of the mechanism and pathological condition of PCO, surgical methods and devices for preventing PCO are discussed in detail. Abstract: Posterior capsule opacification (PCO) is a vision-threatening complication that develops post-cataract surgery in 100% of dogs. The capsular tension ring (CTR) is an intracapsular device implanted during cataract surgery that decreases human PCO clinically. The purposes of the present study were to determine the CTR effect on canine lens epithelial cell (LEC) proliferation and migration in an ex vivo model and on clinical PCO in canine cataract patients and also to determine the safety of canine CTR use. The ex vivo study revealed that CTR and IOL implantation together decreased LEC migration and LEC density on the posterior lens capsule up to day 14 of culture in comparison with single implant or control capsules. In vivo, the CTR does not statistically significantly decrease the extent of PCO up to 4 months post-cataract surgery, and there is no statistically greater risk of intra- or post-operative complications in eyes receiving CTRs.

"Purpose: Posterior capsule opacification (PCO) is the most common complication after cataract surgery, affecting up to 50% of patients 2-5 years post-operatively. Epithelial-to-mesenchymal transition (EMT) is the main pathophysiology underlying PCO. Metformin, a drug with an excellent safety profile used primarily for diabetes, has been shown to suppress EMT. The objective of this study was to test the effectiveness of metformin in inhibiting EMT in an in vitro model of PCO. Methods: The human lens epithelial cell (LEC) line HLE-B3 was exposed to transforming growth factor-beta (TGF-[beta]) and fibroblast growth factor (FGF) to induce EMT. Subsequently, the effect of metformin on the following cellular parameters were determined: (1) survival, using a viability assay and drug concentrations ranging from 0-100 mM; (2) expression of epithelial (Pax6, E-cadherin) and mesenchymal ([alpha]-smooth muscle actin or [alpha]-SMA, fibronectin) markers via Western blot; (3) morphology, via microscopy and image analysis; (4) and migration, using the wound assay. The presence of the metformin uptake receptor SLC22A1 in the cell line and the ratio of active to inactive protein kinase B (pAkt/Akt) were assessed via Western blot. SLC22A1 expression was confirmed with immunohistochemistry on donor eyes. Statistical analysis of variance (ANOVA) with Tukey post-hoc test was done for analysis of cytotoxicity, morphology and migration data.
