Les envenimations par serpents marins en Nouvelle-Calédonie sont rares mais potentiellement graves, susceptibles d'engager à très court terme le pronostic vital. L'objectif principal de cette étude était de faire une synthèse des données épidémiologiques, cliniques et thérapeutiques se rapportant aux patients examinés au Centre Hospitalier Territorial de Nouvelle-Calédonie (CHT) pour morsure par serpent marin, avec pour finalité de comparer les résultats obtenus avec les données de la littérature. Nous avons inclus rétrospectivement les patients ayant été examinés au CHT entre le 1er janvier 2000 et le 30 juin 2016, ayant pour diagnostic final une morsure par serpent marin. Sur 27 cas de morsure inclus, 7 cas d'envenimation vraie ont été recensés dont 3 cas de rhabdomyolyse isolée, 1 cas de curarisation isolée et 3 cas d'envenimation mixte avec curarisation et rhabdomyolyse. Quatre patients ont été pris en charge en pré-hospitalier par l'équipe du Service Mobile d'Urgence et de Réanimation (SMUR). Le pronostic vital a été engagé pour 3 cas et 4 patients ont reçu une immunothérapie antivenimeuse. La recherche d'une curarisation est primordiale devant un risque important de mise en jeu du pronostic vital. La fréquence des complications secondaires à une rhabdomyolyse dans notre étude a été moindre. Toutefois le nombre de cas de rhabdomyolyse, plus important dans notre série que dans celles de la littérature, et leur caractère parfois asymptomatique justifient leur recherche systématique. La létalité potentielle de ces envenimations, d'apparition parfois brutale, impose de promouvoir une prise en charge urgente avec un appel systématique du Centre 15.

L’objectif principal de cette thèse est de décrire et comprendre l’écologie de serpents marins : les tricots rayés de Nouvelle-Calédonie. Ces serpents marins sont très particuliers : ils sont amphibies et dépendent à la fois du milieu marins pour se nourrir mais aussi fortement du milieu terrestre pour toutes les autres activités (digestion, mue, accouplement, ponte,...). L’utilisation de deux milieu de vie contrastés offre des opportunités rares pour comprendre comment l’écologie de ces animaux s’ajuste aux contraintes (synergiques ou antagonistes) qu’imposent les milieux terrestre et marin. La présence de deux espèces proches de tricots rayés vivant en sympatrie dans le lagon de Nouvelle-Calédonie (Laticauda laticaudata – le tricot rayé bleu et L. saintgironsi – le tricot rayé jaune) permet une approche comparative pour étudier l’écologie particulière de ces animaux, mais aussi pour comprendre les mécanismes permettant leur coexistence.
