These articles, embracing the results of extensive reading, as well as my own studies and observa tions, give a fairly comprehensive View of a subject, which has not received much attention in English medical litera ture. I have thought that the publication of these papers in book form would make them more accessible to those interested in the subject. For several reasons, it has been thought advisable to publish the papers in much the same form as they originally appeared in the columns of the Lancet, and therefore a certain amount of repetition, nu avoidable in the circumstances, will be found in each. With a clear understanding of this definition there is nothing misleading about the term, which is a most convenient one for describing a group of cases, which, however, includes several different forms. All the varieties have this point in common, that the inability to interpret written and printed language is not dependent upon any ocular defect, but upon disorder of the cerebral visual centres. The case just recorded presents many striking points of difference from the case recorded by me in the preceding chapter. Both, however, belong to the group which embraces disorders of the cerebral visual centres, and hence the present case has been described as a peculiar form or varietv of word-blindness. Complete word-blindness involving absolute inability to interpret written or printed language (alexia) is due, as was pointed out in the preceding chapter, either to a lesion in the visual memory centre, which occupies the left supra-marginal and angular gyri, or to a lesion which divides completely the fibres connecting this centre with the visual perceptive centres occupying both occipital lobes chiefly in the neighbourhood of the cuneus and calcarine fissure. But if the visual memory centre be itself intact and the conductivity of the connecting fibres be only partially impaired there may not be absolute inability to read (alexia), but there may be very great difficulty in interpreting written or printed symbols (dyslexia). The concept of Minimal Brain Dysfunction (a previous term for ADHD) has had a tumultuous, and some would say, checkered history. Originally published in 1981, this title was one of a series of volumes dealing with specific developmental problems in children whose mothers registered for prenatal care in the Collaborative Perinatal Project (NCPP) of the National Institute of Neurological and Communicative Disorders and Stroke (NINCDS). In this volume, relationships between symptoms of minimal brain dysfunction and more than 300 prenatal and postnatal variables are examined in a cohort of nearly 30,000 7-year-old children. Despite greater understanding more recently, diagnosis and treatment continue to cause controversy. This is an early investigation into the concept of MBD and its causes, today it can be read in its historical context.
