This dissertation develops an investigation into psychiatry that is both theoretical and pragmatic. The research critically assesses conventional psychiatry, promotes personal and professional development, and aims to enhance clinical practice. My thesis is that a systematic engagement with integral methodological pluralism (IMP) can be used to assist psychiatrists and other psychiatric providers as well as psychiatric providers-in-training, to develop greater wisdom, compassion, and skillful means. There are several major aspects to this metatheoretical research study. First, I review and draw from fields such as critical and liberatory psychiatry, metaintegral and integral theories, and the history of psychiatric paradigms to show the need for psychiatric providers to engage issues of mental health more integrally. Second, I explicate and use IMP to engage several emergent fields in psychiatry: postcolonial psychiatry, psychedelic psychiatry, neurodiversity, and ecopsychiatry. These fields help bring balance to conventional Western psychiatry, which is currently biased toward an individual, pathological, biological, and normative agenda. Third, I conclude this dissertation with a discussion of the outcomes as well as a critical assessment of the research. This work has several underlying themes. Mental healthcare providers can develop a more comprehensive understanding of mental health issues, along with an increased ability to respond to suffering, by engaging diverse perspectives. A related theme that emerged through the course of this research was that attending to provider and patient development explicitly has value in the context of mental healthcare. In my efforts to develop a more integral and liberatory psychiatric practice, and in my work with Indigenous people, it became increasingly clear that postcolonial and neocolonial issues needed to be addressed, and this was another recurring theme throughout the study.This study is of particular interest to practicing psychiatrists as well as students and other providers of mental healthcare who are interested in personal and professional growth beyond the bounds of conventional Western psychiatry. This work may also be of interest to other healthcare professionals (including clinicians, administrators, and policymakers), individuals seeking or considering psychiatric care, and anyone interested in understanding more about an integral approach to psychiatry and mental health.

Psychiatry has lagged behind many clinical specialties in recognizing the importance of narrative for understanding and effectively treating disease. With this book, Bradley Lewis makes the challenging and compelling case that psychiatrists need to promote the significance of narrative in their practice as well. Originally called mad-doctoring, psychiatry began in the seventeenth century with the establishing of madhouses and the legal empowering of doctors to incarcerate persons denominated as insane. Until the end of the nineteenth century, every relationship between psychiatrist and patient was based on domination and coercion, as between master and slave. Psychiatry, its emblem the state mental hospital, was a part of the public sphere, the sphere of coercion.The advent of private psychotherapy, at the end of the nineteenth century, split psychiatry in two: some patients continued to be the involuntary inmates of state hospitals; others became the voluntary patients of privately practicing psychotherapists. Psychotherapy was officially defined as a type of medical treatment, but actually was a secular-medical version of the cure of souls. Relationships between therapist and patient, Thomas Szasz argues, was based on cooperation and contract, as is relationships between employer and employee, or, between clergyman and parishioner. Psychotherapy, its emblem the therapist's office, was a part of the private sphere, the contract.Through most of the twentieth century, psychiatry was a house divided-half-slave, and half-free. During the past few decades, psychiatry became united again: all relations between psychiatrists and patients, regardless of the nature of the interaction between them, are now based on actual or potential coercion. This situation is the result of two major ""reforms"" that deprive therapist and patient alike of the freedom to contract with one another: Therapists now have a double duty: they must protect all mental patients-involuntary and voluntary, hospitalized or outpatient, incompetent or competent-from themselves. It is only in the past 20 years that the concept of 'recovery' from mental health has been more widely considered and researched. Before then, it was generally considered that 'stability' was the best that anyone suffering from a mental disorder could hope for. But now it is recognised that, throughout their mental illness, many patients develop new beliefs, feelings, values, attitudes, and ways of dealing with their disorder. The notion of recovery from mental illness is thus rapidly being accepted and is inserting more hope into mainstream psychiatry and other parts of the mental health care system around the world. Yet, in spite of conceptual and other challenges that this notion raises, including a variety of interpretations, there is scarcely any systematic philosophical discussion of it. This book is unique in addressing philosophical issues - including conceptual challenges and opportunities - raised by the notion of recovery of people with mental illness. Such recovery - particularly in relation to serious mental illness such as schizophrenia - is often not about cure and can mean different things to different people. For example, it can mean symptom alleviation, ability to work, or the striving toward mental well-being (with or without symptoms). The book addresses these different meanings and their philosophical grounds, bringing to the fore perspectives of people with mental illness and their families as well as perspectives of philosophers, mental health care providers and researchers, among others. The important new work will contribute to further research, reflective practice and policy making in relation to the recovery of people with mental illness.It is essential reading for philosophers of health, psychiatrists, and other mental care providers, as well as policy makers.
