Mental, neurological, and substance use disorders are common, highly disabling, and associated with significant premature mortality. The impact of these disorders on the social and economic well-being of individuals, families, and societies is large, growing, and underestimated. Despite this burden, these disorders have been systematically neglected, particularly in low- and middle-income countries, with pitifully small contributions to scaling up cost-effective prevention and treatment strategies. Systematically compiling the substantial existing knowledge to address this inequity is the central goal of this volume. This evidence-base can help policy makers in resource-constrained settings as they prioritize programs and interventions to address these disorders.

Since 2005, when the Department of Health introduced its National Service Framework for Long-term Conditions, people with neurological conditions have had better access to health services; but key indicators of quality - such as the rate of emergency hospital readmissions - have worsened. The Department does not know what the Framework and additional spending of nearly 40 per cent over four years have achieved. The Framework was designed to improve care for people with neurological conditions but progress in implementing it has been poor. Access to health services for people with long-term neurological conditions, such as Parkinson's disease, multiple sclerosis and motor neurone disease, has improved and waiting times for inpatient and outpatient neurology have decreased since 2007. The number of elective neurological operations performed has also increased. However, the rate of emergency admissions to hospital has increased significantly and there is large variation in emergency admission rates between Primary Care Trusts. Access to services for patients also varies significantly depending on where they live. Many patients, following their diagnosis, are not given information on their condition, about local services or on available support. Ongoing care is fragmented and poorly coordinated and there is a pattern of patients being referred to hospital for treatment, then discharged and then referred to hospital again. The NAO also found that there is poor coordination between health and social services. The number of adults with a physical disability receiving social services has fallen since 2005-06, because fewer people are now eligible for these services

Principles and Practice of Restorative Neurology is a collection of topics in clinical neurology where progress through research has brought concepts of patient management. The collection emphasizes research done clinically and in experimental laboratories and attempts to discuss restorative neurology in the larger context, encompassing biology, neurobiology, and rehabilitative engineering. The book discusses the concept that neurology should be more therapeutic instead of being just diagnostic because of advances in techniques and drugs. An epidemiology of disability and the evaluation of restorative neurology are explained. The text also presents the developments in the pharmacotherapy of major neurological disorders, such as Parkinson's disease, dementia, and stroke. The use of biomechanical or rehabilitation engineering to treat impaired or lost functions in a patient is compared; the feedback mechanisms and the kinds of treatment including microsurgery, nuerostimulation methods, surgery, and molecular genetics that are applied to specific disorders are discussed extensively. The book also reviews the approaches of gene correction for disorders in the central nervous system. Finally, the future needs of neurology in terms of diagnosis and treatments are dealt with as more research, techniques, and drugs are being discovered and proved useful. The collection is intended for neurologists, neurologists in training, and other physicians whose work lies with neurological disorders. The book can also prove useful for practitioners and academicians in the fields of neurosurgery, internal medicine, psychiatry, and rehabilitation medicine.

This text creates a framework for the integration of urological care into the long-term management of patients with progressive neurological conditions. It begins with a general review of the neuroanatomy and physiology of the bladder, followed by a discussion of common techniques for evaluating bladder and upper tract function, and the general principles of bladder management in patients with neurogenic bladder disease. Section II narrows the scope of each chapter to focus on specific neurological disorders such as Parkinson's Disease, Alzheimer's Disease, Dementia, and Cerebral Palsy. Each chapter begins with a brief synopsis of the neurologic basis of each disease, followed by its most common urologic manifestations, specific recommendations for urologic care in each disease state, and finally a recommended pathway for integrated long-term care of these patients based on available evidence and expert opinion. The final section of the text reviews the generalized care of patients with advanced disease, including palliative and end of life options, focusing on urologic interventions. Urological Care for Patients with Progressive Neurological Conditions will be an invaluable resource for urologists, neurologists, and all health care providers treating patients with neurological diseases.
