Humans have experimented with blood transfusions for over 300 years and have attempted to preserve human blood since the early 1900s. The first modern attempts to store blood were stimulated by World War I when blood was stored in citrate-glucose solutions (Robertson 1918; Rous and Turner 1916). During World War II, the increased need for blood plasma and whole blood resulted in the development of a solution called acid-citrate-dextrose (ACD0 for 21-day refrigerated storage of blood. A slight variation of ACD, called CPD, was introduced in the late 1950s. For CPD, phosphate was added to the citrate and dextrose, which slightly improved the viability of stored red cells, although the dating period was held to 21 days. Blood preservation solutions remained unchanged until the late 1970s when adenine was first added to CPD to produce CPDA-1, which extended the shelf life of blood to 35 days (Peck et al. 1981). CPDA-1 appears to be the industry's final attempt to modify the anticoagulant solution for better blood preservation. However, the success of U.S. and European blood banks with CPDA-1 has encouraged the development of modern additive solutions for component-specific preservation.

The process of freezing and storing red blood cells has been developed into a relatively simple and practical technic. The storage of frozen red blood cells has been approved by the FDA for up to 10 years of storage time. However once the blood is thawed and reconstituted, the red cells can be stored in a normal blood bank refrigerator for a period of only 72 hrs after which they must be discarded. This short storage period creates great problems in the military environment as well as in civilian life. The reasons for this short period of storage are usually stated as follows: A. The thawed red cells have been suspended in a saline preservative medium with a small amount of added glucose which has limited nutritive value. Over a period of time the red cells deplete the nutrients and begin to autolyze; B. Some of the storage media used for longer term preservation are not isoosmotic with red cells and thus produce some destruction of these cells due to hypotonic shock when they are placed in this medium during the storage period; C. As the red cells metabolize the glucose, the pH slowly drops. This decrease in pH causes a decreased viability of the red cells and decrease in the intermediary metabolites necessary for Rbc function; D. Mos of the methods of washing cells employ equipment that is not completely sealed and therefore there is a possibility that bacteria could be introduced into the blood and grow during the storage period. The purpose of this study was to determine if adding one of several currently acceptable anticoagulants to the blood after washing would increase their survival during storage. (ttl).

C. storage procedure that preserves red cell quality and prolongs post-transfusion in vivo survival is described. The improved in vivo survival and the preservation of adenosine triphosphate levels, along with reduction in hemolysis and membrane vesicle production of red blood cells stored at 4.degree. C. for prolonged periods of time, is achieved by reducing the oxygen level therein at the time of storage; in particular, by flushing the cells with an inert gas, and storing them in an aqueous solution which includes adenine, dextrose, mannitol, citrate ion, and dihydrogen phosphate ion, but no sodium chloride, in an oxygen-permeable container which is located in an oxygen-free environment containing oxygen-scavenging materials.
