Abstract: The nutritional status of the pregnant women is being recognized as one of the most important factors in the development of a baby, both pre and postnatally. Malnutrition can cause sterility, spontaneous aborption, stillbirth, premature birth, deformities and mental retardation. Statistics from many sources, such as European clinics during WWII and current figures from hospital observations all over the country, have been collected and compiled to provide information on prenatal and early childhood development in relation to nutrition in three ways--physiologically, neurologically, and behaviorally. Low birth weight is the measurement most closely associated with the incidence of neonatal disorders, and is shown to be closely related also to maternal nutrition. Reproductive casualties, particularly in regard to handicapped children, can be greatly decreased by better education and nourishment of expectant mothers.

How to prevent and manage low birth weight Growth and nutrition during the fetal period and the first 24 months after birth are important determinants of development in early childhood. Optimal nutrition and health care of both the mother and infant during these first 1000 days of an infant's life are closely linked to growth, learning potential and neurodevelopment, in turn affecting long-term outcomes. Children with low birth weight do not only include premature babies, but also those with intrauterine growth restrictions who consequently have a very high risk of developing metabolic syndrome in the future. Epidemiology, epigenetic programming, the correct nutrition strategy and monitoring of outcomes are thus looked at carefully in this book. More specifically, two important nutritional issues are dealt with in depth: The first being the prevention of low birth weight, starting with the health of adolescent girls, through the pre-pregnancy and pregnancy stages and ending with lactation. The second point of focus concerns the nutritional follow-up and feeding opportunities in relation to dietary requirements of children with low birth weight.

In pregnancy, maternal nutrition sustains and nourishes the developing child. Imbalances in either the direction of nutritional excess or deficiency can have adverse consequences for child health. In addition, more research now suggests that good pregnancy nutrition influences child health beyond pregnancy and delivery. This includes modifying the risk of child health outcomes as they enter childhood and adulthood through influences on placental development, hormonal pathways, and organ structure and function. Poor pregnancy nutrition may also compromise maternal health during pregnancy, which may have long-term consequences for women’s health. Understanding the biological and social mechanisms operating during pregnancy can help in the design of better clinical and public health interventions. This Special Issue on “The Role of Pregnancy Nutrition in Maternal and Offspring Health” includes etiological and mechanistic studies of pregnancy nutrition with short- and long-term maternal and child health outcomes, including original research, narrative reviews, and systematic reviews and meta-analyses. Together, this body of work provides important insights into the influence of dietary patterns, food groups, and nutrients on pregnancy outcomes, and long-term neurodevelopmental, respiratory, and metabolic health in the children. Maternal nutrition influences a child’s birthweight, which affects the child’s growth and subsequent survival. However, the broad consequences of maternal undernutrition and the outcomes of interventions to improve maternal nutrition take years to manifest. To examine the long-term health outcomes of low birthweight infants in response to a maternal nutritional supplementation intervention without this obstacle, we developed the Forecasting Population Progress (FPOP) microsimulation model. The intervention we assessed was based on the findings of a published clinical trial outcome that reduced the incidence of low birthweight, a known cause of stunting. We implemented the “before intervention” and “after intervention” simulations and generated the difference in outcomes, using a spatially explicit synthetic baseline population of Indonesia generated from a microdata sample of the Indonesian 2010 census. We focused specifically on two provinces—Yogyakarta and Bali—which represent different levels of fertility and mortality, but both exhibit significant underweight birth. The baseline scenario represented the current nutritional status of pregnant women in the two Indonesian provinces and projected that implementing a multiple nutrition supplementation intervention would, after 30 years, avert 8 per 1,000 low birthweight births, 3.8 per 1,000 stunted children younger than 5 years of age, and 0.25 infant deaths per 1,000 births. As our model results demonstrate, improvement in maternal nutrition would reduce infant mortality, but an even greater impact could be the reduction in growth stunting.

In this final report on the 'The influence of maternal, fetal and child nutrition on the development of chronic disease in later life', the Subgroup on Maternal and Child Nutrition (SMCN) reviews the evidence that early life nutrition exerts long-term effects and influences the risk of chronic disease in adulthood, bringing together findings from both observational and experimental studies, and considering implications for maternal and child nutrition in the UK. SACN concludes that there is cause for concern about the later health consequences of compromised or excessive nutrient supply during early fetal and infant life. It notes that in the context of reproduction, the impact of current dietary patterns on women and girls is of particular concern, and considers that improving the nutritional status of women of childbearing age, infants and young children has the potential to improve the health of future generations. The Committee also offers recommendations for future research in this area, particularly emphasising the need for data which better characterise dietary patterns and patterns of pre and postnatal growth.
