The Global Burden of Disease Study (GBD) is one of the largest-scale research collaborations in global health, distilling a wide range of health information to provide estimates and projections for more than 350 diseases, injuries, and risk factors in 195 countries. Its results are a critical tool informing researchers, policy-makers, and others working to promote health around the globe. A study like the GBD is, of course, extremely complex from an empirical perspective. But it also raises a large number of complex ethical and philosophical questions that have been explored in a series of collaborations over the past twenty years among epidemiologists, philosophers, economists, and policy scholars. The essays in this volume address issues of current and urgent concern to the GBD and other epidemiological studies, including rival understandings of causation, the aggregation of complex health data, temporal discounting, age-weighting, and the valuation of health states. The volume concludes with a set of chapters discussing how epidemiological data should and should not be used. Better appreciating the philosophical dimensions of a study like the GBD can make possible a more sophisticated interpretation of its results, and it can improve epidemiological studies in the future, so that they are better suited to produce results that can help us to improve global health.

CONCLUSIONS the global burden of disease Despite an uneven and sometimes flawed database, there are clear and profound differences in cause-of-death structures between developed and developing regions. Of the 50 million deaths worldwide each year, 39 million (78 percent) are in developing countries. Communicable, maternal, and perinatal causes account for 40 percent of those deaths, whereas these causes account for only 5 percent of deaths in developed regions. Injuries cause roughly equal proportions of deaths in both developed and developing countries and are invariably more common in males than in females. Their relative importance in cause-of-death structures varies, from greater importance in Eastern Europe, Latin America, and China to lesser importance in developed countries. Major challenges in the developing world are inherent in the disease burden on children ages 0 to 14 and are well known: perinatal conditions, diarrhea, acute respiratory infections, measles, malaria, tetanus, and pertussis. A worldwide collaborative effort to measure the impact of health problems on people, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) is coordinated by the Institute for Health Metrics and Evaluation at the University of Washington in Seattle. Data for the project are collected and analyzed by 1,870 collaborators from 124 countries and three territories. GBD uncovers the toll of early death and disability caused by more than 300 diseases and injuries in 195 countries and territories from 1990 to the present. Its methods and metrics emphasize comparability among causes that kill and causes that disable. The GBD is frequently updated using new data to improve past and present estimates of disease burden. The GBD 2015 study shows that, from 1990 to 2015, the world as a whole has been undergoing an epidemiological transition. The nature of that transition is discussed in this document in a brief overview of some key GBD 2015 results.
