Throughout the last decade, the field of clinical psychology has expanded dramatically. Clinical psychologists are involved in the treatment and research of a wider range of problems and disorders than they have ever been before. Evidence has been rapidly ac cumulating regarding the role of psychological variables and stress in the etiology and maintenance of a range of medical and psychiatric disorders. New models of psy chotherapy have been developed and refined, and the specific efficacy of psychother apeutic interventions for an increasing number of disorders (or sUbtypes of disorders) has been documented. However, concurrent with research that demonstrates the impact of psychosomatic factors in various disorders and the efficacy of psychological or psychosocial interven tions, dramatic progress has been made with regard to the investigation of biological factors that may mediate certain disorders. That physical factors may underlie many in stances of psychiatric illness has been repeatedly demonstrated. Also, the efficacy of so matic treatments for different disorders, or for subtypes of disorders, has been reported with increasing methodological rigor.

The understanding of how to reduce risk factors for mental disorders has expanded remarkably as a result of recent scientific advances. This study, mandated by Congress, reviews those advances in the context of current research and provides a targeted definition of prevention and a conceptual framework that emphasizes risk reduction. Highlighting opportunities for and barriers to interventions, the book draws on successful models for the prevention of cardiovascular disease, injuries, and smoking. In addition, it reviews the risk factors associated with Alzheimer's disease, schizophrenia, alcohol abuse and dependence, depressive disorders, and conduct disorders and evaluates current illustrative prevention programs. The models and examination provide a framework for the design, application, and evaluation of interventions intended to prevent mental disorders and the transfer of knowledge about prevention from research to clinical practice. Children living in poverty are more likely to have mental health problems, and their conditions are more likely to be severe. Of the approximately 1.3 million children who were recipients of Supplemental Security Income (SSI) disability benefits in 2013, about 50% were disabled primarily due to a mental disorder. An increase in the number of children who are recipients of SSI benefits due to mental disorders has been observed through several decades of the program beginning in 1985 and continuing through 2010. Nevertheless, less than 1% of children in the United States are recipients of SSI disability benefits for a mental disorder. At the request of the Social Security Administration, Mental Disorders and Disability Among Low-Income Children compares national trends in the number of children with mental disorders with the trends in the number of children receiving benefits from the SSI program, and describes the possible factors that may contribute to any differences between the two groups. This report provides an overview of the current status of the diagnosis and treatment of mental disorders, and the levels of impairment in the U.S. population under age 18. The report focuses on 6 mental disorders, chosen due to their prevalence and the severity of disability attributed to those disorders within the SSI disability program: attention-deficit/hyperactivity disorder, oppositional defiant disorder/conduct disorder, autism spectrum disorder, intellectual disability, learning disabilities, and mood disorders. While this report is not a comprehensive discussion of these disorders, Mental Disorders and Disability Among Low-Income Children provides the best currently available information regarding demographics, diagnosis, treatment, and expectations for the disorder time course - both the natural course and under treatment.

Explanations of abnormal behavior that emphasize the importance of physiological determinants of disorder are relatively unpopular among psychologists, especially among those who work as clinicians in an ap~ plied setting. The reasons for this are theoretical and historical, as well as practical. Physiology and its associated biological disciplines of bio~ chemistry, pharmacology, and genetics are traditionally more associated with medicine; their use to underpin explanations and treatments of behavioral abnormality has consequently demanded knowledge to which most psychologists are not exposed and skills that are unavailable to them. The dichotomy thus created between medical and psychologi~ cal approaches has caused many psychologists to disregard physiologi~ cal factors. Even when the latter are recognized as important, many psychologists have been unwilling to admit to the fact, in the belief that by doing so they will commit themselves to an overly medical model of psychological disorder, undermining what they see as preferred views of abnormality. As I have become increasingly aware in following the progress of this book, in the United States the theoretical issues in this debate have been further sharpened by professional rivalries (present but less explicit on the European scene from which I write) between medical and nonmedical health care workers, regarding facilities for and approaches to the treatment of the mentally disturbed. Faced with these divisions of interest, psychologists have available two courses of action.
