From Editor’s Desk Perhaps in no other situation a physician experiences the same magnitude of agony and ecstasy as while attending an emergency. Though initially one tends to be ‘down in the dumps’ after being unsuccessful in a case or being euphoric over a successful management of a dire emergency, in due course one develops an attitude of equanimity. This attitude is essential so that our own emotions do not affect our management plans. I have chosen two aphorisms in Medicine, ‘Primum non nocere’ and ‘Primum succurere’. While attending to a patient, if we are not able to cure him or relieve him it is excusable but we have to do our best not to harm the patient in the process and if we happen to be in the vicinity of any one in need of urgent medical attention, we shall hasten to attend to him, are the respective connotations of these aphorisms. Sometimes, we have hesitation in attending to a patient in need of medical attention when the given situation does not belong to our expertise lest we err in the management, like a physician at the site of a road traffic accident. The Supreme Court has ruled that a medical professional should not hesitate on such grounds and must try to provide whatever care he can provide in such cases. That exactly is what ‘Primum succurere’ means. Emergencies in medicine include many sub specialities but to restrict the number of pages and the size of the Monograph, we are concentrating mainly on topics we face in our steady clinical practice as physicians. I have attempted to present the management of the medical emergencies in different scenarios, such as situations where no assistance or special tools are available to situation where all types of assistance and tools are available as in an intensive care unit. We have also included topics on the role of alternative medicine in emergency handling of cases. This emergency medicine, a new branch of medicine has become so indispensable presently that the MCI (Medical Council of India) and its BOG (Board of Governors) in their notification on November 14, 2019 state as follows. “No MBBS course nod to medical colleges in the country without emergency department (ED)”. The measures call for all medical colleges to have freely functional ED as per the MCI norms by 31th march 2022, adding that all new medical colleges seeking LOP (Letter of Permission) w.e.f. 2021—2022 MBBS batch should ensure that a freely functional ED is in existence as per the MCI norms. Infectious diseases are caused by microorganisms that release toxins or invade body tissues. The most common pathogenic organisms are bacteria, viruses, fungi, protozoa, and helminths. Systemic infections usually cause fevers, chills, sweats, malaise, and occasionally headache, muscle and joint pain, or changes in mental status, and even septic shock-MODS. Infectious diseases have always threatened populations and caused great loss of life in history, but since the last century, with the discovery of antibiotics, historical trends have been reversed. It is reported that between 1990 and 2017, age-standardized disability-adjusted life year (DALY) rates decreased by 41.3% (38.8-43.5) for infectious diseases. However, in 2017, lower respiratory infections are still ranked as the third cause of DALYs. Drug therapy is the one of most critical management strategies for infectious diseases. Efficacy and safety of drug therapy should always be considered, especially in elders, neonates and immunosuppressed patients. Off-label drug therapy has been performed for complex infectious diseases, especially for new emerging infectious diseases. However, off-label drug therapy can be extremely complex: some are prone to rational use, while others are more susceptible to the issues of irrational use. With the increasing rate of publication of data in this area, new evidence for the efficacy and safety of different treatment approaches is constantly developing. Thus, rigorous analysis of such data is imperative, which will guide future clinical practice and guidelines.

Every DSM-5 diagnosis includes an exclusion criterion that the disorder is not better explained by a medical condition. Meeting this criterion can be difficult for a variety of reasons. The psychiatric signs and symptoms of medical disorders are not commonly emphasized in medical textbooks. Further, illness scripts for medical diagnoses do not often overlap with psychiatric disorders, making it difficult to know what medical conditions should be ruled out. For example, irritability is a common symptom in polycystic ovarian disorder, but PCOS is rarely on the differential for irritability. Similarly, while hypothyroidism is commonly linked to the illness script of depression, patients with MDD may be just as likely to have diabetes - an infrequently considered diagnosis for depression. “Buzzword” medical conditions that are commonly prioritized in medical student training can negatively influence classic illness scripts. While such diagnostic possibilities make for good multiple-choice questions, they are frequently rare and may inadvertently undermine important common possibilities. For example, a patient with chest pain in the context of anxiety is more likely to have asthma, acute coronary syndrome, or even a pulmonary embolism than pheochromocytoma. In a recent white paper issued by the American Psychiatric Association, it has urged psychiatrists to better advocate for patients with severe mental illness who often lack access to primary care. But some psychiatrists may be unfamiliar with physical exam maneuvers and medical review of systems (ROS) questions. Complex medical systems may delegate the physical exam to physicians outside of psychiatry, or there may be a temptation to rely on the emergency room’s “medical clearance” as a “medical rule-out.” Both can result in decreased familiarity with physical exam techniques previously mastered as part of medical school. A cursory review of the physical exam maneuvers and concise symptom-based medical ROS lists can alleviate some of these concerns. This book is intended to provide psychiatrists and physicians who routinely evaluate psychiatric symptoms with the tools needed to rule out medical conditions that could be causing those symptoms. It will start with an introduction that reviews why the text is needed and potential larger gaps in training that might contribute to the necessity for such a text. Each chapter thereafter will focus on a specific symptom. Each symptom will be defined to ensure accuracy. Then a differential of common medical conditions that can cause that psychiatric symptom will be provided. For each diagnosis key history, physical exam, laboratory, and radiologic findings will be provided that help rule the condition out. Screening tools that can help rule out medical etiologies will also be provided. Where available, positive predictive values (PPVs) will be provided to help users understand the likelihood that a negative finding or result indicated that a medical disorder is not present. While individual aspects of this text exist in other formats, the comprehensive nature of our approach, descriptions of psychiatric symptoms to means of ruling out potential medical etiologies, is not currently available to providers. This text will assist providers in ruling out medical etiologies of common psychiatric symptoms, ensuring patients are diagnosed correctly. Such an improvement has the potential to dramatically improve patient outcomes.
