Sylhet, the area of Bangladesh most closely associated with overseas migration, has seen an increase in remittances sent home from abroad, introducing new inequalities. Social change has also been mediated by the global forces of Western biomedicine and orthodox Islam. This book examines the effects of these modernizing trends on mental health and on local, traditional healing as the new inequalities have exacerbated existing social tensions and led to increased vulnerability to mental illness. It is the young women of Sylhet who are most affected. The global economy has increased competition for resources and led to marriage being seen as a route to economic advancement. Parents prefer to give their daughters in marriage to families that will widen their social contacts and enhance their economic and social standing. Accordingly, the young wife's outsider status (and hence vulnerability to mental illness) has increased as it is no longer customary to give daughters in marriage to local kin. Yet, patients and their families do not work out tensions passively. They are active agents in the construction of their own diagnosis. The extent to which patients act or are acted upon is an investigation that runs throughout the book. Alyson Callan is a psychiatrist and anthropologist. She currently works as a consultant psychiatrist in Brent for the Central and North West London NHS Foundation Trust.

This book is about mental health in Bangladesh. Bangladesh is a densely populated country in South Asia with a population of about 170 million. It has seen significant economic growth over the last decades, and it has recently improved from being a low-income country to a lower middle-income country. Currently, Bangladesh is facing a double burden of disease, i.e., both communicable and non-communicable. About 60% of the disease burden is incured by non-communicable diseases. Mental disorders are one of the top five burdens of non-communicable disease in the country. However, psychiatry is a neglected issue in Bangladesh. There are high stigma, services gaps, out-of-pocket expense, low mental health literacy, and extreme scarcity of budget. Academically, it has also been under-addressed and under-researched. Therefore, the editor aims to provide a comprehensive book on mental health in Bangladesh, based on existing evidence and expertise, focusing on academic aspects of community mental health service.

This book is about access to mental health care in South Asia. South Asia consists of eight countries with low and middle-income backgrounds. The region contains a combined population of about 2 billion, making up about a quarter of the global population. The people of this region share common cultures, beliefs, and behavioral patterns regarding physical and mental health. Among them, about 15% (about 300 million) have been suffering from common mental disorders. However, there is a persisting high treatment gap for mental illness in the region. Hence, despite having a mental illness, only a small percentage of the population is able to have access to essential mental healthcare. Though governments are trying to bridge the gap by improving mental health policies and programs, it is still a major challenge delivering mental healthcare to all people in need. Due to the income category and dual disease burden of the countries, there are some additional enduring challenges like poor funding and research, inadequate and inequitable manpower, huge out-of-pocket expenses, poor mental health literacy, income disparity, and high stigma. This book aims to highlight the issues related to accessing mental health services in a densely populated low and middle-income setting. This book is the first of its kind in comparing and contrasting the delivery status of mental health services in South Asian countries.
