On April 23, 1906, Alexis Carrel presented his manuscript entitled "The Surgery of Blood Vessels" to the Johns Hopkins Medical Society. His work on the tech nique of small vessel anastomosis and the transplantation of visceral organs earned him the Nobel Prize in 1912. The use of magnification in operative surgery began in 1921 with the work of Nylen, who first employed the microscope for reconstructive procedures in the middle ear. Jacobson and Suarez published their initial observa tions on the use of microsurgery for the anastomosis of small vessels in 1960, and two years later, Malt reported the historic replantation of an upper extremity. To surgeons involved in reconstruction of the extremities, the evolution of micro surgery has provided the most significant advances of the past three decades. The dramatic clinical successes of replantation surgery and free tissue transfer have substantially improved functional and cosmetic results in addition to decreas ing morbidity in patients who present with complex reconstructive problems. Mi crosurgical Reconstruction of the Extremities crosses the traditional surgical discip lines and will be invaluable to orthopaedic surgeons, plastic surgeons, hand sur geons, and general surgeons, as well as to residents and fellows.

This is no doubt a must-read and must-have book." – from the Foreword by Fu-Chan Wei, MD Adequate evaluation of lower limb wounds for salvage requires an itemized assessment of vascular, osseous, soft tissue, and functional deficits. Lower Extremity Reconstruction: A Practical Guide by renowned reconstructive surgeons and perforator flap masters J.P. Hong and Geoffrey G. Hallock presents an orthoplastic approach to this growing and challenging area of microsurgery. Throughout the well-illustrated text and videos, an impressive cadre of international surgeons share pearls and insights, including esoteric knowledge and step-by-step demonstration of techniques with pertinent case examples. This unique guide presents a practical, visual, and stepwise approach to learning and mastering a full array of flap and microsurgery approaches for traumatic, dysvascular, metabolic, and oncologic lower limb defects. Organized into 26 topic-specific chapters, the book covers a full spectrum of lower extremity topics—from wound prep, timing, closure alternatives, and therapy, to soft-tissue tumors and a new concept in drop foot treatment. Numerous videos demonstrate how surgeons can leverage workhorse options to prevent chronic non-healing wounds or amputations and achieve the goal of limb salvage. Key Highlights Lower extremity soft-tissue reconstruction techniques using local muscle and perforator workhorse flaps Bone salvage and restoration techniques, including vascularized bone grafts Diabetic foot management with in-depth discussion of the SCIP flap and perforator-to-perforator concept Rationale for partial and subtotal foot amputation 27 videos and more than 600 illustrations enhance understanding of microsurgical interventions With insights from top microsurgeons on how to achieve the best outcomes for patients with lower limb defects, this is a must-have resource for plastic and orthopaedic surgeons, especially specialists who treat patients at trauma centers.

Advances made in microsurgical techniques during the last three decades have dramatically improved the treatment of complex limb injuries, such as a closed fracture covered by healthy tissue and a complex open fracture of the tibia. Microvascular transfers from the foot have revolutionized the treatment of digital amputations, and vascularized bone transfers are used in various indications. The microvascular techniques are still evolving. Many imaginative new flaps are being proposed to meet precisely the functional requirements of the various defects. Alternative anastomotic techniques are being investigated and the no-flow phenomenon has been extensively studied. Allotransplantation may be feasible in the future. This unique book is a result of the exchange of ideas among clinicians of various specialties noted for their innovations and basic researchers involved with the problems of microvascular reconstruction of upper and lower extremities. While most of the chapters are invited contributions, some are based on presentations made during an international symposium held in Brussels, Belgium, in 1997.

Microsurgical techniques are widely used in the field of orthopaedics. This book addresses all aspects of orthopaedic microsurgery, from development of the principles to their applications. Replantation of fingers, hands and extremity parts are well summarized by highly experienced microsurgical surgeons. The methods for tissue transplantation and microsurgical reconstruction of tissue defects, which have been proved to be successful tools for saving severely injured extremities, were developed by experienced orthopaedic surgeons in cooperation with plastic surgeons. Injuries to peripheral nerves and the brachial plexus are also discussed extensively in this monograph. All illustrations and tables were meticulously selected and are easy to understand. The book was written for all microsurgeons who work in the fields of orthopaedics, plastic and hand surgery. Path-Breaking Work in Plastic and Reconstructive Microsurgery by Dr. Doina Dumitrescu-Ionescu was reviewed by David Soutar for The Royal College of Surgeons of England: “This is a refreshingly original and unusual text. It is a testament to the life work of the author and her struggle to develop clinical microsurgery in Rumania. It takes the reader on a journey starting with learning the basic techniques of microsurgery, laboratory and experimental research, teaching and training. It then progresses to the building of an infrastructure for development and clinical practice, the maintenance of meticulous records and the evaluation of results. As such, the book is a very personal account written in an informal style so that the reader can follow the author’s own development and experience. “The format is unusual beginning with the author’s philosophy for developing reconstructive microsurgery. It then covers personal techniques developed for peripheral techniques developed for peripheral nerve repairs and microvascular anastomosis. It is clear from early on in this book that the author’s main interest lies in the management of peripheral nerves, particularly in the limbs. There is a section on reconstructive microsurgery of the brachial plexus following trauma and a section on limb replantation.” Says the author, “The reconstructive microsurgery experience helped me to modernize reparative plastic surgery in Romania in reconstructive plastic surgery and reconstructive microsurgery by radically changing the surgical approach.” This allowed also for new procedures in Dupuytren’s Disease, Volkmann, and post-burn contractures approach, “revolutionizing everything that was repair surgery of any extremity, cephalic, superior, inferior, but also the extension of reconstructions in the large defects of suprascheletal soft parts.”
