Atherosclerosis is the most common causes of vascular diseases.So;looking for the physical factors and correlations that explain the phenomena of existence the atherosclerosis disease in LAD artery in some people rather than the others is achieved in this study by analysis data from coronary angiography as well as estimating the blood velocity from coronary angiography scans without having a required data on velocity by using some mathematical equations and physical laws. It found some information that may explain the ability of small arteries to develop the lesions with time mainly in proximal site of LAD artery. For investigation the fluid-structural response to pulsatile Newtonian and non-Newtonian blood flow through an axisymmetric stenosed coronary artery;the simulation is built up by using ANSYS11 to evaluate the biomechanical parameters in the atherosclerotic process. Rigid and Newtonian cases were investigated to provide an understanding on the effects of incorporating Fluid-Structural Interaction (FSI) into model. non-Newtonian Carreau model was investigated with FSI. For validation of the models and methods used, the computation results are compared with previous studies.

The pathological complications of atherosclerosis, namely heart disease and stroke, remain the leading cause of mortality in the world. Cardiovascular illness is highly prevalent among the American population. One manifesto of cardiovascular disease is Peripheral Artery Disease (PAD) which is caused by atherosclerosis in the arteries. Atherosclerosis is a vascular disease that reduces arterial lumen size through plaque deposition and arterial wall thickening. The flow patterns in the arteries are highly modulating along with the cardiac cycle and a strong function of the waveform created by the heartbeat. In this study, a computational simulation model is developed to analyze blood flow distribution in a femoral artery network based on Navier-Stokes equation. A comparative analysis of blood flow through femoral artery is done based on Newtonian and non-Newtonian blood flow by using Carreau-Yasuda model subjected to a waveform based on a cardiac cycle. The non-Newtonian CFD flow analysis model is use to analyze blood flow distributions in a femoral artery along with the adjacent capillary porous tissue medium. This simulation model is used to analyze the flow and stress field in a healthy and atherosclerosis affected femoral arteries. Velocity, pressure and Wall Shear Stress fields over cardiac cycle are analyzed to demonstrate that adverse flow field created upstream and downstream of the blockage that may cause enhanced growth in size of the blockage.

Cardiovascular diseases (CVDs) are among the leading causes of death in the world. In this study, an attempt was made to model the flow dynamics of blood in abnormally narrowed artery. Finite volume solver FLUENT was used for the analysis with the aim of understanding the consequences of increasing the degree of stenosis using a two-equation turbulence model. The compliant nature of the artery was neglected, and Newtonian behavior of the blood flow was assumed for the larger arteries. Steady-flow simulations with 75% area reductions were used to establish the validity of the current models by employing the standard and transitional variant of the k -w turbulence models. Subsequently, it was found that transitional k -w model was suitable for the low Reynolds number internal flows associated with the transition to turbulence, although only a minor departure in terms of the turbulence intensity peak was observed. Unsteady blood flow was introduced by employing a sinusoidal pulsatile waveform at the inlet. The pulsatile nature of the blood flow was investigated in the range of the constriction ratio from 60% to 90%, with an inlet-specified pulse. It was hypothesized that the severity of the stenosis played a major role in the initiation of the turbulence, since no major turbulence was reported for the 60% and 75% area reductions, while increasing the constriction ratio of 90% significantly altered the flow dynamics and triggered the transition to turbulence much earlier than anticipated. The outcome of current numerical efforts was expressed in terms of wall shear stress, a hemodynamically relevant parameter.

Cardiologists must answer three important questions when evaluating and treating patients with a coronary artery stenosis. As a physiologist: "What is the effect of this stenosis on coronary blood flow and myocardial function?"; as a clinician: " Is this lesion responsible for the patient's symptoms?"; and finally as an interventionalist: "Will revascularization of this artery improve the patient?" Fundamentally, the answer to these questions can be given to a large extent by measuring coronary pressure. That is the rationale of writing this book. 1. 1 Historical overview. Andreas Gruentzig and most interventional cardiologists in the early days of PTCA, had the intuitive feeling that pressure measurements could help to establish the severity of a coronary stenosis and to monitor the progress and result of a coronary intervention. At that time, measuring coronary pressure by the balloon catheter was part of a standard procedure. A residual transstenotic gradient of less than 15 mmHg was generally considered as a good result. Later, however, it turned out that measuring these (resting) gradients with balloon catheters was inaccurate an only had a limited prognostic value. Moreover, because there was no consistent theory to correlate pressure measurements to blood flow, the interest in measuring coronary pressures faded and disappeared almost completely with the introduction of new balloon catheters not intended for pressure measurement.

"Myocardial infarction (MI) is a severe form of heart disease caused by the blockage of coronary arteries. Percutaneous coronary angioplasty of coronary artery bifurcation is associated with complications. The Medina classification is used by physicians to categorize the bifurcation lesions. However, the Medina classification is an anatomical classification which does not discriminate the complex disease. For example, there are reported cases, which are still unexplained, implying that there is anatomically significant appearance of a coronary stenosis in bifurcation disease detected by coronary angiography, intravascular ultrasound measurement (IVUS) and quantitative coronary angiography (QCA) which does not correlate with standard fractional flow reserve (FFR) measurement (Ben-Dor et al. 2011, Koo et al. 2005, Sarno et al. 2009, Meijboom et al. 2008, Ziaee et al. 2004). As another problem, there are some reports that patients may develop clinically significant myocardial ischemia from coronary artery steal (Kosar et al. 2009). The functional state of a patient with stenosis can be determined by coronary flow, pressure and other biomechanical parameters like wall shear stress (WSS). The characterization of physiological flow on the basis of functional and biomechanical parameters is essential to assess the intuitiveness of the Medina classification and to be able to identify the riskier disease to employ the most effective treatment strategy. However, the impacts of the bifurcation angle, location of stenoses and their percentages have not systematically been studied in stenosed bifurcation with respect to a classification scheme. To this end, computational models of normal and stenosed coronary artery bifurcations are developed and physiological flow are studied. The effect of percentage of stenosis, lesion length and location is determined aiming at deriving a functionally base classification scheme that could resolve the above mentioned clinical observations. The results of this work showed that in true bifurcation lesions, the lower blood flow in the Side Branch (SB) creates a critical condition for this branch in bifurcation lesion (1, 0, 1) compared to bifurcation lesions (1, 1, 1) and (0, 1, 1) respectively. It should be noted that bifurcation lesion (1, 0, 1) corresponds to the steal condition. Occlusion of the SB is an important contributing factor in increasing the MI rate. Our results demonstrated that the non-stenotic main branch distal steals flow away from the artery in the presence of a proximal main branch and side branch stenoses. Stenosed bifurcation (1, 0, 1) leads to increase flow disturbance in the SB associated with low WSS. Examination of degree and location of stenoses and the SB angle showed the degrees of stenosis and the plaque location have more effect on the blood flow ratio and WSS distribution. A new classification scheme was proposed to make it functional as a global scheme for the assessment of coronary bifurcation lesions.Treatment of coronary bifurcation lesions is mostly performed by stent implantation. The key in the treatment of bifurcations lesion is the SB. Several stenting techniques are used for bifurcation stenting. Direct comparisons of different strategies and their outcomes remain limited. In this work computational models of normal and stented coronary artery bifurcations are developed and the influence of two stent configurations on alteration of WSS in the stented vessel is studied. Comparison of two stenting techniques showed that the deployment of a stent in the SB results in higher values of WSS distal to the main branch; which may help reduce the development of the restenosis." --
