Although there has been a slow but steady decrease in incidence, gastric cancer remains the second leading cause of cancer death worldwide. Several aspects of the oncological and surgical management are still controversial and so gastric cancer represents a challenge for the surgeon. This book aims to delineate the state of the art in the surgical and oncological treatment of gastric cancer, describing the new TNM staging system, the extent of visceral resection and lymphadenectomy focusing on the different open and minimally invasive surgical techniques and discussing intraoperative chemohyperthermia and neoadjuvant and adjuvant treatment. Operative endoscopy and endoscopic ultrasonography are also discussed, as these now have an important role in both diagnostic work-up and palliative care of gastric cancer patients. Recent decades have seen remarkable advances in the treatment of upper gastrointestinal malignancies, i.e., adenocarcinoma and squamous cell carcinoma as well as gastrointestinal stromal and other rare tumors of the esophagus and stomach. While, historically, surgical resection has been the sole treatment for these tumors, multimodal therapies have meanwhile proven their efficacy. At present, pre- and postoperative chemotherapy and radiotherapy, targeted drug therapy, and stage-specific surgical approaches are all indispensable cornerstones of an individualized treatment for upper gastrointestinal malignancies. Gastric cancer is one of the most common and potentially lethal gastrointestinal malignancy, but it is now at the forefront of modern oncology due to new discoveries. In recent years, the field of gastric cancer has been transformed by many notable developments in both the diagnostic area and the therapeutic strategies. The main aim of the text is to propose a comprehensive, state-of-the art review of this field, and will serve as a valuable resource for oncologists, pathologists, surgeons and researchers with an interest in gastric cancer. The first chapters will review the interplay between pathogens and especially H. pylori, inflammation, genomic instability, and tumorigenesis. Secondly, an exhaustive overview of the diagnosis of gastric cancer from a clinical/endoscopic and pathologic perspectives will be illustrated. Two specific chapters will deal with tissue and serum biomarkers with an important role in diagnosis as well as prognostic and predictive markers of response to specific therapy. A central section of the book will focus on treatment options available today for patients. Surgery is the cornerstone of treatment for gastric cancer; however, the prognosis of patients with locally advanced disease is still poor even after curative resection. Therefore, in order to improve the prognosis of patients, especially with advanced stage, a multimodal strategy is advisable and is thoroughly addressed in the appropriate section. With the advent of next-generation sequencing and new preclinical model technologies, the genomic landscape of gastric cancer has been delineated and molecular characterizations finalized to novel therapeutic targets of each molecular subtype have been identified. These advances are making it feasible to integrate clinical and phenotype-based diagnostic and therapeutic methods and are described in a dedicated section. The text will conclude with chapters focusing on the future direction of gastric cancer research in the precision medicine era: an in-depth analysis of non-coding RNAs and their role in gastric cancer, the impact of immunomodulation/immunotherapy and the perspectives of nano-medicine will be finally proposed to the audience. A brief review of the existing literature addressing the above mentioned topics will be present in each chapter. Gastric cancer has been one of the great malignant scourges affecting man kind for as long as medical records have been kept. Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.

Stomach cancer (also called gastric cancer) is the growth of cancer cells in the lining and wall of the stomach. Stomach cancer represents roughly 2 percent (25,500) cases of all new cancer cases yearly in the United States, but it is much more common in Japan, Great Britain, South America, and Iceland. It is associated with high salt in the diet, smoking, and low intake of fruits and vegetables.Infection with H. pylori is the main risk factor in about 80 percent or more of gastric cancers. It is more common in men. A very small percentage of diffuse-type gastric cancers are thought to be genetic. Hereditary Diffuse Gastric Cancer (HDGC) has only recently been identified and research is ongoing, but genetic testing and treatment options are available for families at risk. Metastasis occurs in 80-90 percent of individuals with stomach cancer, with a five year survival rate of 75 percent in those diagnosed in early stages and less than 30 percent of those diagnosed in late stages.
