Diffuse optical tomography (DOT) is an emerging noninvasive functional imaging method for breast cancer diagnosis and neoadjuvant chemotherapy monitoring. In particular, the multimodal approach of combining DOT with x-ray digital breast tomosynthesis (DBT) is especially synergistic as DBT prior information can be used to enhance the DOT reconstruction. DOT, in turn, provides a functional information overlay onto the mammographic images, increasing sensitivity and specificity to cancer pathology. We describe a dynamic DOT apparatus designed for tight integration with commercial DBT scanners and providing a fast (1 Hz+) image acquisition rate to enable tracking of hemodynamic changes induced by the mammographic breast compression. The most significant advance enabling fast acquisition was the design and construction of a direct analog-to-digital conversion frequency-domain near-infrared spectroscopy (FD-NIRS) component. It achieves simultaneous dual wavelength operation at 685 nm and 830 nm by concurrent 67.5 MHz and 75 MHz frequency modulation of each laser source, respectively, followed by digitization using a high-speed analog to digital converter and real-time hybrid FPGA-assisted demodulation by discrete Fourier transform (DFT). The overall DOT system integrates 96 CW-NIRS and 24 FD-NIRS source locations, as well as 32 CW-NIRS and 20 FD-NIRS detection locations into low-profile plates that mate to the DBT compression paddle and x-ray detector cover, respectively. The plates and the embedded optical fibers are made of plastic to minimize x-ray absorption and thus allow true simultaneous acquisition of the DBT image. We first characterize each major system component individually, and then demonstrate overall performance using static and dynamic tissue-like phantoms, as well as in vivo images acquired from the pool of patients recalled for breast biopsies at the Massachusetts General Hospital Breast Imaging Division.

Abstract: Non-muscle-invasive bladder cancer affects millions of people worldwide, resulting in significant discomfort to the patient and potential death. Today, cystoscopy is the gold standard for bladder cancer assessment, using white light endoscopy to detect tumor suspected lesion areas, followed by resection of these areas and subsequent histopathological evaluation. Not only does the pathological examination take days, but due to the invasive nature, the performed biopsy can result in significant harm to the patient. Nowadays, optical modalities, such as optical coherence tomography (OCT) and Raman spectroscopy (RS), have proven to detect cancer in real time and can provide more detailed clinical information of a lesion, e.g. its penetration depth (stage) and the differentiation of the cells (grade). In this paper, we present an ex vivo study performed with a combined piezoelectric tube-based OCT-probe and fiber optic RS-probe imaging system that allows large field-of-view imaging of bladder biopsies, using both modalities and co-registered visualization, detection and grading of cancerous bladder lesions. In the present study, 119 examined biopsies were characterized, showing that fiber-optic based OCT provides a sensitivity of 78% and a specificity of 69% for the detection of non-muscle-invasive bladder cancer, while RS, on the other hand, provides a sensitivity of 81% and a specificity of 61% for the grading of low- and high-grade tissues. Moreover, the study shows that a piezoelectric tube-based OCT probe can have significant endurance, suitable for future long-lasting in vivo applications. These results also indicate that combined OCT and RS fiber probe-based characterization offers an exciting possibility for label-free and morpho-chemical optical biopsies for bladder cancer diagnostics

A new optical imaging modality has been developed for small animal in vivo imaging of near-infrared fluorescence resulting from fluorescent contrast agents specifically targeted to molecular markers of cancer. The imaging system is comprised of an intensified charge-coupled device (ICCD) for the detection of ultra-low levels of re-emitted fluorescence following the delivery of an expanded beam of excitation light. The design of the ICCD detection system allows for both continuous wave (CW) and frequency-domain modes of operation. Since the accurate acquisition of frequency-domain photon migration (FDPM) data is important for tomographic imaging, the imaging system was also validated using experimentally obtained FDPM measurements of homogenous turbid media and diffusion theory to obtain estimates of the optical properties characteristic of the media. The experiments demonstrated that the absorption and reduced scattering coefficients are determined least accurately when relative measurements of average light intensity I rel [over] DC are employed either alone or in a combination with relative modulation amplitude data I rel [over] AC and/or relative phase shift data [theta] [superscript]rel. However, when FDPM measurements of [theta] [superscript]rel are employed either alone or in combination with I rel [over] AC data, the absorption and reduced scattering coefficients may be found accurate to within 15% and 11%, respectively, of the values obtained from standard single-pixel measurements; a result that suggests that FDPM data obtained from an ICCD detection system may in fact be useful in tomographic imaging. Furthermore, intensified-detection allows for sub-second exposure times, permitting the acquisition of dynamic fluorescence images immediately following administration of the contrast agent. Experimental results demonstrate that when coupled with a suitable pharmacokinetic model describing targeted dye distribution throughout the body, dynamic fluorescence imaging may be used to discriminate spontaneous canine adenocarcinoma from normal mammary tissue. A separate experiment demonstrates that pharmacokinetic analysis of dynamic fluorescence images enables one to estimate the rate constant governing Kaposi's sarcoma tumor uptake of an [alpha subscriptV beta]3 integrin-targeted dye and integrin receptor turnover rate. The rate constant for uptake was calculated to be 0.16-sec−1 while the turnover rate of the integrin receptor was estimated to occur within 24-hours.
