Der Band nimmt die sozialen und gesellschaftlichen Dimensionen der aktuellen Veränderungen im deutschen Krankenhauses in den soziologischen Blick. Betrachtet werden einerseits Transformationsprozesse auf der Ebene von Organisationen und Professionen, andererseits aber auch ihr Zusammenhang mit allgemeineren gesellschaftlichen und politischen Entwicklungen. How does the market affect and redefine healthcare? The marketisation of Western healthcare systems has now proceeded well into its fourth decade. But the nature and meaning of the phenomenon has become increasingly opaque amidst changing discourses, policies and institutional structures. Moreover, ethics has become focussed on dealing with individual, clinical decisions and neglectful of the political economy which shapes healthcare. This interdisciplinary volume approaches marketisation by exploring the debates underlying the contemporary situation and by introducing reconstructive and reparative discourses. The first part explores contrary interpretations of ‘marketisation’ on a systemic level, with a view to organisational-ethical formation and the role of healthcare ethics. The second part presents the marketisation of healthcare at the level of policy-making, discusses the ethical ramifications of specific marketisation measures and considers the possibility of reconciling market forces with a covenantal understanding of healthcare. The final part examines healthcare workers’ and ethicists’ personal moral standing in a marketised healthcare system, with a view to preserving and enriching virtue, empathy and compassion. The nursing process generally is understood as key element of professional nursing care in Germany. This study follows this argument back to the introduction of the nursing process in the 1970s and 1980s. At that time, the German healthcare system underwent dramatic changes and economic reorganization, which can be understood as the emergence of the neoliberal rationale in Germany. The argument of cost explosion was used to restructure hospitals into enterprises that were to operate based on the logic of the market. Its cybernetic logic made the nursing process an ideal instrument to restructure nursing care. Perspectives of governmentality and critical accounting reveal the nursing process as an accounting tool which has made nursing calculable. And while German nurses valued its potential for professionalization, the findings suggest that a newly constituted accountable nursing vocation can instead be considered as de-professionalizing.

Public and political interest in social entrepreneurship (SE) is increasing while it remains a contested and ambiguous concept. Philipp Kenel traces the popular media representation of SE in Germany over time (1999—2021), making an important empirical contribution to the sociological and political understanding of the phenomenon. He shows that until 2008, SE was mainly understood as a reform of the welfare infrastructure (including public and non-profit institutions). From 2009, SE was increasingly conceptualised as part of the economy, while sometimes challenging and other times reaffirming mainstream economic logics. More recently, in somewhat competing perspectives, SE has been framed as part of the ›start-up‹ world or as a force for deeper social and ecological transformation.

The organizational requirements for an error-free and effective care of patients within a large-scale hospital are high: despite all efforts to improve the process quality and the establishment of risk management systems and reporting systems, there exists the general impression that the processes and communicative challenges are still – in particular with regard to the grown influence of economic factor – prone to error: in case of doubt to the harm of the patients. The topic, which has a medical and organizational sociological, an economic and not least a (criminal-)legal dimension, was the subject of the traditional annual meeting of the Göttingen Institute for Criminal Science in July 2016.

Die Politik betreibt sodann im Kontext der gemeinsamen Selbstverwaltung eine symbolische Politik des Integrationsfonds.
