As part of the health policy of a country, national medicine policy should be an essential section which aimed at rational use of pharmaceuticals. World Health Organization supports country to develop, implement and monitor the effectiveness of national medicines policy’s guidelines, strategies and plans. A good and effective national medicine policy will ensure the availability, affordability, accessibility and rational usability of essential medicine, and traditional, complementary and alternative medicine. National medicine policy will make sure that medicines in the country are not only safe, effective and of good quality, but they must also be cost effective. When this book was written, Malaysia still has not had any comprehensive document on the national medicine policy. A study was conducted to evaluate the status of the pharmaceutical sector in relation to the national medicine policy components. This book provides information about the status of medicine policy in the country and to what extent the medicine policy makers recognize and are aware of the existence of medicine policy in the country. How can we expect for effective and reliable policies, if policy makers do not know or are not sure about the national medicine policy and the various medicine policies available in the country! The book is designed for academicians, practitioners, students, as well as policy makers in the health care institutions. The separation of dispensing from doctor or what is simply called dispensing separation is a holty debated health policy proposition in Malaysia but rarely discussed. Each time the issue resurfaced, emotional opposition took place and extinguish the discussion of an important policy change that is long overdue in Malaysia. Consequently, both policy markers and the public are deprived of rational arguments for the policy. This book reported an economic evaluation study that has evaluated the benefit of dispensing separation policy from a Malaysian societal perspective using a cost-benefit analysis (CBA) framework. In the absence of empirical data or controlled trial on the policy, the study has also applied a decision modeling approach that make use of empirical data from published studies. This is the first time suach an approach was used in addressing this issue and we feel that this would have appeal not only to Malaysian policy, but also to other countries that are also contemplating such change.

Penyakit tidak berjangkit (NCD) ialah punca utama morbiditi dan mortaliti di seluruh dunia. Di Malaysia, NCD seperti penyakit kardiovaskular, kanser, penyakit sistem pernafasan kronik, diabetes dan penyakit tidak berjangkit yang lain menyumbang kepada 74.0% daripada jumlah kematian pada tahun 2016. Sebagai faktor risiko dapat diubah yang utama bagi NCD, penyakit yang berkaitan dengan tabiat merokok adalah antara empat punca utama kematian di Malaysia. Selain itu, kekurangan aktiviti fizikal dan amalan pemakanan yang tidak sihat juga merupakan faktor risiko dapat diubah bagi NCD yang dikaitkan dengan pelbagai jenis penyakit kronik. Tambahan pula, kedua-dua faktor ketidakaktifan daripada segi fizikal dan amalan pemakanan yang tidak sihat juga menyumbang kepada masalah berat badan berlebihan atau obesiti yang sekali gus menjadi faktor risiko utama bagi NCD yang berkaitan dengan berat badan. Bagi kaum wanita pula, kurangnya kesedaran tentang kanser payudara dan kanser serviks menjadi faktor risiko utama bagi kedua-dua jenis penyakit kanser tersebut, yang paling biasa dihidapi dan menjadi penyebab kematian dalam kalangan wanita di Malaysia. Hakikatnya, kanser-kanser tersebut dapat dicegah dan disembuhkan jika dikesan pada peringkat awal. Sistem penjagaan kesihatan awam di Malaysia juga menghadapi beban ekonomi yang berat disebabkan NCD yang sering dihidapi rakyat Malaysia. Kerajaan Malaysia membelanjakan kira-kira RM3 bilion setahun untuk merawat penyakit yang berkaitan dengan tabiat merokok. Selain itu, kos rawatan bagi obesiti dianggarkan berjumlah RM6.4 bilion setahun, manakala kos rawatan bagi kanser payudara atau kanser serviks pula dianggarkan berjumlah RM51.6 juta setahun. Maka, jelaslah bahawa dalam usaha menjana pembangunan yang pesat ke arah mencapai status negara maju menjelang tahun 2020, Malaysia seharusnya bersedia menganggarkan tanggungan kos kesihatan dan kos ekonomi yang amat tinggi akibat NCD. Rawatan NCD mungkin akan membebankan sistem penjagaan kesihatan Malaysia, malah cenderung mengalihkan sumber ekonomi yang terhad daripada perubatan pencegahan dan rawatan penyakit-penyakit berjangkit (seperti kolera, radang paru-paru, demam denggi, malaria dan HIV/AIDS). Secara optimistik, banyak beban NCD dapat dielakkan dan kerugian daripada segi ekonomi dapat dikurangkan jika langkah pencegahan yang sistematik dapat diambil pada peringkat permulaan dan perkembangannya. Hal ini kerana kebanyakan faktor risiko NCD merupakan tingkah laku yang dapat diubah dan dipengaruhi faktor penentu sosioekonomi, sekali gus menjadikan faktor-faktor tersebut sebagai punca dan kesan penyakit-penyakit ini. Oleh itu, kebanyakan NCD dapat dicegah atau permulaannya dapat ditangguhkan sekiranya penentu sosioekonomi yang dikaitkan dengan faktor-faktor risiko yang dapat diubah ini dikenal pasti dan ditangani sewajarnya.
