Outcome research has shown that the experience of a rupture-repair process in the therapeutic relationship can have a beneficial impact upon symptomology, interpersonal problems and social functioning. There is a lack of qualitative research on therapeutic ruptures and how they are repaired. This study explored parallel accounts of therapeutic ruptures produced by clients and therapists during long-term psychodynamic therapy. Interviews were conducted with four client-therapist dyads. The data were analysed using Interpretative Phenomenological Analysis. Five overarching themes and six sub-themes are presented. These included: clients' experience of the danger of emotional experience (Emotions as dangerous); accounts of the discovery during therapy being a difficult and gradual experience (Therapeutic discovery; gradual and hard work; to and fro); the experience of problems within the therapeutic relationship (The struggle; not knowing; control and power); followed by a connection within the relationship (The positive connection; emotional sensitivity; shining a light); and the experience of the end of the therapy (Leaving and being left). The findings are discussed in relation to the literature on the emotional experience of psychodynamic therapy. The results demonstrated the importance of attunement and reciprocity within the therapeutic relationship to help maintain a positive connection in the dyads.

This study examined client-reported changes in the therapeutic relationship across 29 sessions, spanning ten cases of short-term, Process-Experiential psychotherapy. Cases and sessions were empirically identified, based on clients' end-of-session ratings on the Working Alliance Inventory (WAI). This empirical selection strategy was intentionally broad and atheoretical, to encompass a variety of change scenarios. Therapist and client behaviors and interactions during selected sessions were explored using the Structural Analysis of Social Behavior (SASB) and the Hill Counselor and Client Verbal Response Modes (VRM) Systems. Both frequency and sequential analysis of category codes were conducted, followed by a session-by-session review of the ten cases. Observed therapist and client behaviors clearly reflected the Process-Experiential therapy model as well as therapist efforts to establish and maintain empathic attunement. No significant differences were found in the frequencies of SASB or VRM codes between strongest and weakest alliance sessions or between rupture and post-rupture sessions. This raises questions about the ability of these measures to differentiate between subtle in-session processes. The case reviews underscored the clients' independent and often-overlooked contributions to the formation and repair of the therapeutic relationship.

A strong therapeutic alliance throughout psychotherapy consistently predicts positive treatments outcomes (Horvath et al., 2011), but less is known about the within-session process of rupture and repair. Ruptures are not good or bad, but how they are managed or mismanaged is predictive of outcomes (Safran et al., 1990). There are mixed findings regarding type (withdrawal or confrontation) and intensity of ruptures and treatment outcomes (Boritz et al., 2018; Muran et al., 2009; Eubanks et al., 2019; Schenk et al., Rubel et al., 2018). The current study evaluated if therapist interventions predicted changes in rupture intensity and patient productivity. These variables were rated on an archival sample of 10 full-recorded analyses between 1968 and 2011. The mean age of the participants was 27 years and 50% were female. The Rupture Resolution Rating System (3RS; Eubanks et al., 2015) was used to measure rupture intensity, and the Analytic Process Scale (APS; Waldron et al., 2004) was used to measure intervention quality and patient productivity. Raters were unable to achieve acceptable interrater reliability for withdrawal ruptures. Intervention quality did not significantly predict changes in confrontation rupture intensity or changes in patient productivity. Ruptures likely vary in how quickly they are resolved, as we did not find significant moment-to-moment changes in rupture intensity and patient productivity. In this way, time may be an important aspect of repair, and should be considered in future studies.
