This book reviews the recent international experience with the applications of computer assisted orthopaedic surgery in clinical practice. Recent decades of the human condition have witnessed the dramatic evolution of technology and the application to everyday existence. The ability to use such innovation in surgical practice is now easily within our grasp. Though clinical experience is short term, as will be demonstrated the problems are finite and limited only by the need for refinement. We can now clearly state that current surgical practice will be revolutionized by these new methodologies. This edition is all encompassing for musculoskeletal surgery including the spine, trauma, sports, and reconstructive surgery. Because of its simplicity, computer navigation will be an early tool in such areas as total joint replacement, anterior cruciate ligament reconstruction, and placement of pedicle screws in complex spinal surgery. New techniques in Minimally Invasive Surgery will require the precision and digital "surgical exposure" offered by the computer. However, we anticipate in a few years, that robotics with computer activated technology will rise to an important role for the practicing surgeon. Surgeons who are comfortable with technology and yearn for better solutions with their techniques will benefit from the knowledge of this experience. Americans at the AAOS 2003 annual meeting in New Orleans, have now joined the wave of enthusiasm for computer assisted orthopaedic surgery (CAOS) as well as the exciting new vistas of Minimally Invasive Surgery.

Orthopaedic surgery is urr c ently just a few moments past the sunrise of computer implementation for the per formance of surgery itself. OUT pe s cialty has always seemed to have a nearly unique. over arching characteristic -it deals almost exclusively with alteration, typically rebuilding or reconstituting. structure. What we do has more of a structural impact than a physiologic one, even when compared to the structural alterations introduced by vascular and cardiac surgery_ This structural reconstitution contrasts strongly with the largely excisional haracteri c stic of most other ur sgical fields. The relevance of Ihis feature is thai OUf specialty, as much as any, and more than most, needs precision of cuning, alignment, and replacemenl. As we contrast the rudimentary tools, we use in our operating rooms with the sophis ticated robotic, computerized, and more extensively mechanized equipment of industry, we seem very far behind. We are almost ancient considering the pace of development and change over the last fifty years. Many question the need, practicality, expense and therefore ultimate acceptance of these methods into ortho paedics. However, the quality I mention above, leads me to believe that eventually, hi-tech involving computer assis tance and including robotic tools, will almost take over. The current era is and will be appreciated as a beginning a time when new equipment and techniques are introduced, bugs worked out, and valuable tools refined. nearly perfected, with costs lowered.

The reader is enthusiastically encouraged to tackle this second edition text in two ways. The first is simply to scan chapters with their introductions, summaries and conclusion points. Second, is to delve into those sections of seeming greater interest depending upon one’s s- cialty and role. The expansion and quality of this material speak to the success of the first edition by these editors and many similar authors. In addition, the continued and enlarged interest in computer assisted Orthopedic surgery indicates the relevance and enduring importance of this advance in our field of musculoskeletal surgery. I suggest that no other discipline in surgery is so appropriately suited to computer assistance including robotic performance. Orthopedics has always seemed unique to this author in that it focuses more than any other medical field on gross physical, mechanical structure. We deal nearly exclusively in physical repair of broken elements, rearrangement of deformed ones, and resurfacing or refurbishing those that are diseased in a way that has altered their mechanical integrity, shapes, and other structural aspects.

"Take away my knee pain and give me better motion". This is what the arthritic patient expects from a Total Knee Arthroplasty (TKA). By virtue of standardization of the TKA procedure, surgeons can nowadays solve the pain issue for the majority of the patients. Restoration of function is a goal of a different order and forms the scope of this book. The editors confronted today's leading knee surgeons with the limitations of current surgical techniques and technology. They challenged them to define new thresholds of functional capacity after Total Knee Arthroplasty. "A Guide to Get Better Performance in TKA" describes the cutting edge in surgical techniques, prosthetic design and achievement of excellent function for these patients.

Computer-Assisted Surgery (CAS) is a new tool for performing complex procedures in a predictable and safe way. This book is designed to serve as a comprehensive review of Computer-Assisted Surgery, covering the current status of both research and applications. CAS includes Virtual Preoperative Planning (VPP) and Intraoperative Virtual Navigation (IVN), which are a set of technologies used to measure oncological margins in 3-Dimensions (3D), to locate small intraosseous tumors and apply controlled resections preserving anatomical structures. During VPP, patient acquired multimodal images are processed and an interactive virtual scenario is created. This can then be used as a platform to measure oncological distances and preplan osteotomies in safe areas. IVN is a procedure which allows the execution of the VPP with a mean error of less than 3mm. For the student, medical doctors, research and development scientists or new researchers, the protocols are central to the performance of Computer-Assisted technologies.

Minimally invasive surgery has evolved as an alternative to the traditional approaches in orthopedic surgery and has gathered a great deal of attention. Many surgeons are now p- forming all types of procedures through smaller surgical felds. Along with changes in the surgical technique, there have been rapid advances in computer navigation and robotics as tools to enhance the surgeon’s vision in the limited operative felds. With these new techniques and technologies, we must ensure that these procedures are performed safely and effectively with predictable clinical outcomes. This book has been expanded from our previous publi- tions to include spine and foot and ankle surgery, along with updated sections on knee arth- plasty, hip arthroplasty, and upper extremity surgery. The clinical information and surgical techniques, along with tips and pearls, provided by experts in the feld allows the reader to grasp a comprehensive understanding of the nuances of MIS. It is our intention that this text will be a valuable reference for all orthopedic surgeons. New York, NY Giles R. Scuderi, MD Piscataway, NJ Alfred J. This open access book describes and illustrates the surgical techniques, implants, and technologies used for the purpose of personalized implantation of hip and knee components. This new and flourishing treatment philosophy offers important benefits over conventional systematic techniques, including component positioning appropriate to individual anatomy, improved surgical reproducibility and prosthetic performance, and a reduction in complications. The techniques described in the book aim to reproduce patients’ native anatomy and physiological joint laxity, thereby improving the prosthetic hip/knee kinematics and functional outcomes in the quest of the forgotten joint. They include kinematically aligned total knee/total hip arthroplasty, partial knee replacement, and hip resurfacing. The relevance of available and emerging technological tools for these personalized approaches is also explained, with coverage of, for example, robotics, computer-assisted surgery, and augmented reality. Contributions from surgeons who are considered world leaders in diverse fields of this novel surgical philosophy make this open access book will invaluable to a wide readership, from trainees at all levels to consultants practicing lower limb surgery
