Inhaltsangabe:Zusammenfassung: Seit einigen Jahren befindet sich die Krankenhauslandschaft im Umbruch. Das Ziel ist ein dienstleistungsorientiertes Unternehmen und weniger ein Betrieb der Versorgungsverantwortung, wie er häufig vorzufinden ist. Um eine Umsetzung des genannten Zieles voran zu treiben, bedarf es neu entwickelter Managementmethoden, die effizient einsetzbar sind. Die Wirtschaftlichkeit von Krankenhäusern und Kliniken hatte in früheren Jahren eine untergeordnete Bedeutung, was vor allem an den verschiedenen Finanzierungssystemen lag. Mit dem Krankenhausfinanzierungsgesetz (KHG) 1972 wurde die duale Finanzierung für Krankenhäuser eingeführt. Tragendes Element war eine Trennung der Investitionskosten von den Benutzerkosten. Folge des neu eingeführten Selbstkostenprinzips war ein enormer Investitions- und Kostenschub. Zwischen 1971 und 1975 stiegen die Ausgaben der gesetzlichen Krankenversicherung (GKV) für die Krankenhauspflege um 100 Prozent. Die jährlichen Steigerungsraten lagen zwischen 25 und 30 Prozent. Da ein Drittel aller Ausgaben der GKV auf den Krankenhaussektor entfielen, wurde die Kritik an den hohen Kosten von allen Seiten immer lauter. Mit dem Gesundheits-Strukturgesetz von 1992 wurde das Selbstkostendeckungsprinzip wieder abgeschafft. Neben den Abteilungs- und Basispflegesätzen wurden preisliche Elemente, wie Fallpauschalen und Sonderentgelten geschaffen. Eine weitere Reformierung der Finanzierungssysteme gelang mit dem Gesundheitsreform- Gesetz 1999, das ein neues pauschalierendes Vergütungssystem für allgemeine voll- und teilstationäre Krankenhausleistungen vorsieht. Grundlage bilden die G-DRGs (German Diagnosis Related Groups). Das diagnosebezogene Fallpauschalensystem dient der Vergütung ganzer Behandlungsfälle im stationären Bereich (Ausnahme: Psychiatrie). Für eine Eingruppierung der Patienten in den DRG-Fall sind folgende Kriterien von Bedeutung: Operationsschlüssel, Haupt- und Nebendiagnose, Alter, Geschlecht, Verweildauer, etc. Ziel der DRG-Einführung ist neben der verbesserten Transparenz der Krankenhausleistungen die Ausschöpfung von Wirtschaftlichkeitsreserven. Die erwarteten Folgen der Einführung des neuen Entgeltsystems, wie z.B. Verkürzung der Verweildauer und Verbesserung der betriebswirtschaftlichen Abläufe stellen sehr hohe Anforderungen an die Krankenhäuser.
