Neurological disorders in children are common: families want to know what is wrong, why it happened, and whether it will happen again. Management and treatment depend on establishing the diagnosis, which usually requires investigations, but the number of possible neurological investigations is now very large indeed, and uncritical investigations may be seriously misleading and often costly. This book, based on the authors’ vast combined personal experience, gives practical guidance on how to target any specific condition with the minimum of tests. Part 1 is a brief distillation of advice on clinical history and examination, introducing the problems that beset those who deal with neurological disorders in childhood. Part 2 outlines the investigations available to solve these diagnostic problems. Part 3 takes a problem-oriented approach to the most appropriate investigations in the various clinical scenarios that may present to the practitioner: the starting point is the patient’s presentation, not the diagnosis. Throughout, vignettes of real cases help to illustrate the use of the tests in different clinical situations. The authors co-wrote the hugely popular Handbook of Neurological Investigations (“the Blue Book”) 20 years ago. This new book, while closely following the highly successful approach of the earlier title, is thoroughly up to date, with fresh material, new case vignettes, and additional investigations covered.

In writing this book, I have attempted to provide information about the relative value of particular investigations in various neurological disorders. The book is divided into sections dealing with the methods of investigation, the investigation of particular symptoms, the investigation of specific neurological conditions and neurological emergencies. Finally, the assessment of certain disorders suggesting a particular anatomical localization, for example isolated optic atro phy, is considered. Following an appraisal of the literature, each section ends with a recommendation regarding appropriate investigation. In some in stances this is supported by an algorithm. Specific recommendations have been attempted despite the risk of producing over-dogmatic criteria for patient management. A small amount of illustrative material is included. The importance of good history and physical examination with proper interpretation being adequate for neurological diagnosis in as much as 90% of cases cannot be overemphasized. Investigations however are essential for collaboration with or confirmation of the clinical diagnosis. Recent advances in scientific technology have increased the physician's armamentarium in investigative modalities. The advent of these sophisticated modalities (neuro imaging techniques, biochemical and gene studies) for the investigation of neurological disorders has caused an over reliance on these investigations alone for diagnosis. This practice should be avoided as it certainly does not guarantee the correct diagnosis in neurological patients. The aim of the neurologist is to arrive at a final diagnosis by the intelligent analysis of the clinical data aided by the least number of investigations; relevant. In essence, clinical diagnosis should always precede and guide the investigations. In making a neurological diagnosis therefore, it is pertinent to have a thorough clinical evaluation, localize the lesion, make a clinical diagnosis or narrow down the differential diagnosis before requesting investigations. This book 'Fundamentals of Neurological Diagnosis' gives a simplified step wise approach to the diagnosis of Neurological disorders. The first part is a brief anatomy of the nervous system; the second part is a guide to the localization of lesions while the third part details the basic investigations of the central nervous system.
