Neuropsychologists are being increasingly called upon to demonstrate the value of their services. This edited book introduces clinical neuropsychologists to the concepts and challenges involved in conducting cost outcome research. It provides examples of how such research can be conducted within clinical neuropsychology and therefore is a "beginning" step in what must become an interdisciplinary effort. The text suggests that more than cost effectiveness studies should be considered when demonstrating the clinical utility of neuropsychological services. The concept of "objective" and "subjective" markers of value is emphasized, particularly as it relates to measuring the impact of a neuropsychological examination. Chapters review the economic burdens associated with different neurological conditions commonly seen by neuropsychologists. They also provide examples of how clinical neuropsychological services to different patient populations may reduce "costs" and increase "benefits" and suggest directions for beginning cost outcome research. Furthermore, the book summarizes the utility of various neuropsychological services that may be helpful to readers concerned with healthcare economies. It has long been appreciated among clinical neuropsychologists that both primary and secondary factors contribute to cognitive dysfunction in neurological patients. Primary influences are the direct result of the extent and location of damage to the brain. Secondary influences stem from something associated with brain injury or disease besides the specific areas of the brain affected. For example, a patient with a neurological disease may develop depression, something which in turn often impacts cognitive functioning. Other secondary factors associated with disease besides depression can also negatively impact cognitive functioning, including anxiety, pain, fatigue, and motor impairments, to name a few. Despite the widespread appreciation of the importance of such secondary factors on cognitive functioning in clinical neuropsychology, there has never been a single source for this information that is readily available to clinicians and researchers. The present volume provides such a source, using an evidence-based framework comprised of two broad sections. The first section includes chapters that comprehensively address particular secondary influences, independent of any particular neurological disorder. For example, there are chapters on the impact on cognitive functioning of depression, anxiety, fatigue, pain, diagnosis threat, and symptom invalidity. The second section has chapters that focus on specific neurological conditions and the most salient secondary factors that need to be considered in these conditions. Multiple Sclerosis, HIV, Parkinson's Disease, Traumatic Brain Injury, Alzheimer's Disease/MCI/Stroke, and Epilepsy are all considered. The chapters include vivid case studies that illustrate the principles outlined in the chapters to help clinicians consider how such principles can apply to particular patients. Cluster analysis is a multivariate classification technique that allows for identification of homogenous subgroups within diverse samples based on shared characteristics. In recent years, cluster analysis has been increasingly applied to psychological and neuropsychological variables to address a number of empirical questions. This book provides an overview of cluster analysis, including statistical and methodological considerations in its application to neurobehavioral variables. First, an introduction to cluster analysis is presented that emphasizes issues of relevance to neuropsychological research, including controversies surrounding it use. Cluster analysis is then applied to clinical disorders that do not have an associated prototypical neuropsychological profile, including traumatic brain injury, schizophrenia, and health problems associated with homelessness. In a second application, cluster analysis is used to investigate the course of normal memory development. Finally, cluster analysis is applied to classification of brain injury severity in children and adolescents who sustained traumatic brain injury.
