These eight volumes, which developed out of the international congress "Update and Future Trends in Fibrin Sealing in Surgical and Nonsurgical Fields" held in November 1992, present the state of the art in fibrin sealing. In itially, fibrin sealant played an important role in surgery. During the past few years, it has been increasingly applied nonsurgically, and we can now say that it has become an integral component of medical treatment. The doubts which have been raised by nonusers about the efficacy of fibrin sealant are no longer valid. The correct indication and technique continue to be basic prerequisites for effective treatment. Even today - 20 years after fibrin sealant was first used - the three most prominent effects of fibrin sealant are still hemostasis, sealing of the wound, and support of wound healing. The problems posed by the transmission of viral infections have gained sub stantially in importance because of the potential transmission of AIDS via fi brin sealant. Fortunately, this is so unlikely today that it no longer represents a cause for concern, which does not mean, however, that research in this field can be discontinued.

The region of the skull base was long considered a surgical barrier because of its complex anatomy. With few exceptions, the region immediately beyond the dura or bony skull base constituted a "no man's land" for the surgeon working from the other direction. A major reason for this was the high morbidity associated with operative procedures in that area using traditional dissection techniques. This situation changed with the advent of the operating microscope. Used initially by ear, nose and throat specialists for resective and reconstructive surgery of the petrous bone and parana sal sinuses, the operating microscope was later introduced in other areas, and neurosurgeons began using it in the mid-1960s. With technical equality thus established, the groundwork was laid for taking a new, systematic, and interdisciplinary approach to surgical problems of the skull base. Intensive and systematic cooperation between ear, nose and throat surgeons and neurologic surgeons had its origins in the departments of the University of Mainz bindly supported by our chairmen Prof. Dr. Dr. hc Kurt Schiirmann (Department of Neurosurgery) and Prof. Dr. W. Kley (Depart ment of Ear, Nose and Throat Diseases, Head and Neck Surgery). The sellar region and paranasal sinuses constitute the anatomical sections of the skull base in which pathological entities warrant interdisciplinary management. Processes originating in the paranasal sinuses can reach and involve the skull base in and around the sella, sometimes not respecting the natural dural boundary. On the other hand, lesions involving the sellar block, such as pituitary adenomas and meningiomas, can also extend downwards into the paranasal sinuses. The orbit and cavernous sinus may be subject to involvement and infiltration by both paranasal and sellar pathology. The advancement and new achievements of modern diagnostic procedures, such as high-resolution CT, three-dimensional reconstruc tion, MRI, and MRI angiography, as well as the detailed selective angiographic protocols and endovascular techniques, have increased the possibilities for surgical management of this type of pathology with extra- and intracranial involvement. Fibrin plays a prominent role in wound healing. It has a hemostatic effect, induces cellular response to wound damage, and, by forming strands to build a matrix, assists in neovascularization and fibroblast proliferation. The concept of using clotting substances from human blood for wound manage ment and to achieve hemostasis in bleeding parenchymatous organs can be traced to 1909, when Bergel [1] reported on the hemostatic effect of fibrin powder. In 1915, Grey [3] employed fibrin to control bleeding in neurosurgical operations of the brain. A year later, Harvey [4] used fibrin patches to stop bleeding from parenchy matous organs in general surgery. It took more than two decades for this ingenious idea to be rediscovered. In 1940, Young and Medawar [8] reported on experimental nerve anastomosis by sealing. Similarly, Tarlov and Benjamin [7] reunited nerves with plasma clots in 1943. Tarlov improved the results obtained with clot anastomosing of nerves by avoiding tension at the nerve stumps. In 1944, Cronkite et al. [2] reported on an initial series of eight cases in which fibrinogen and thrombin had been used successfully for anchoring skin grafts.
