Inflammatory bowel disease is a chronic immune-mediated inflammation of the gastrointestinal tract of unknown origin, which includes Crohn's disease, ulcerative colitis, and inflammatory bowel disease of unclassified type. It is associated with different intestinal and extraintestinal manifestations like different neurological and psychiatric disorders. Histology is an important tool in the diagnosis and prognosis of inflammatory bowel disease and has an increasing part in patients' management. The objective of treatment is to make and keep long-lasting remission by immunosuppressive treatment like corticosteroids, thiopurines, and monoclonal antibodies directed against tumor necrosis factor alpha. Therapeutic drug monitoring of thiopurines by measuring levels of their metabolites has been proposed as a potentially effective tool in optimizing therapy in inflammatory bowel disease. Diets and their components influence microbiota of the intestine, function of the epithelial barrier, immune response, and other factors that have an important role in development and treatment of inflammation in the gut mucosa.

In spite of significant scientific progress in recent years, the aetiopathogenesis of chronic inflammatory bowel diseases (IBD) remains elusive. With a prevalence of 1:700 to 1:500 in central Europe, both diseases are not rare. However, most physicians will generally treat only a few patients with IBD in their clinical practice. Because of this limited experience and the variety of clinical IBD manifestations, IBD patients are frequently treated in specialized tertiary care centres. Nevertheless, IBD patients will also consult less specialized general practitioners, who will not be able to focus extensively on the management of IBD patients due to time constraints. The aetiology of the chronic inflammatory bowel diseases - Crohn's disease and ulcerative colitis - is still enigmatic. The therapeutic approach has therefore traditionally focused on anti-inflammatory principles, including corticosteroids and aminosalicylates. Since a significant proportion of patients is steroid-dependent or refractory and because of the problematic side-effects of long-term systemic steroids, active immunosuppression has gained acceptance in the field. The classical immunosuppressants azathioprine and 6-mercaptopurine have long been evidence-based in IBD, but underused. Recently, methotrexate and cyclosporine have also been proven to be effective in certain situations. Newer drugs like tacrolimus, mycophenolate and others may be similarly useful but their potential is still unclear. Finally, the immune modifiers including IL-10 and TNF-antibodies have been successfully subjected to controlled trials. Other experimental drugs discussed in this book are on the horizon. This volume is the Proceedings of Falk Symposium 119 held in Freiburg-im-Breisgau, Germany, October 3-4, 2000, and covers systematically the field of immunosuppression in inflammatory bowel diseases. An international symposium on inflammatory bowel diseases was held in Jerusalem on September 7th-9th, 1981. The symposium was sponsored by the Hebrew University-Hadassah Medical SchooL and the Israeli Gastroenterological Society. The idea was to bring together leading workers in the field, to invite all those interested to present their new work on IBD and thus to enable exchange of information and cross fertilization needed to improve our understanding and handling of these diseases. The symposium was organized into four panels devoted to state of the art reviews. presentation of new findings and approaches on the follow ing topics: New pathological concepts. etiology. pathogenesis and management of IBD. In addition. 89 abstracts were presented as posters during the symposium. all of which were published in the book of abstracts.
