In recent years, there has been rapid growth in knowledge pertaining to the nervous system. This has, in some measure, been due to the development and application of a number of techniques such as the 2-deoxyglucose method and microchemical methods for measuring metabolites and regional cerebral blood flow. Data from the application of these techniques are just beginning to be collected, and the next few years promise to bring many new and exciting findings. The study of energy metabolism in brain is particularly interesting due to the fact that although the brain has scant energy reserves (as compared with the liver), it has one of the highest metabolic rates in the body. Recent studies from several laboratories have shown a surprising divergence of re sponses to metabolic insult in different areas of brain. In this regard, the cer ebellum, for example, may have metabolic features which are uniqve from those of any other region. The high-energy phosphate compounds ATP and phos phocreatine, supplied by the oxidative metabolism of glucose, are necessary for normal cerebral functions such as the maintenance of membrane potentials, transmission of impulses, and synthetic processes. Interruption of substrate or "poisoning" of the system by a variety of means lead to a rapid change in cellular energetics, and ultimately cell death. From the clinical standpoint, an interesting feature of metabolic encephalopathy is that in many cases, early diagnosis and treatment may result in a rapid reversal of symptoms.

On December 19 ,20 and 21 1979, we had the opportunlty to organize a Workshop on "Antenatal factors affecting meta bolic adaptation to extrauterine life - Role of carbohydra tes and energy metabolism". This meeting was made possible thanks to grants from the Committee of Medical Research and Public Health (CRM) of the Commission of European Communi ties. We want to express our warmst gratitude for the effort that has been made and we hope that the exchange of information between the participants will allow better understanding of the mechanism of adaptation to extrauterine life and that the conclusions drawn will contribute to a better care of the newborn. Adaptation to extrauterine life is one of the most impor tant steps in life. At the moment of birth, numerous conditions are drastically changed and in order to survive the newborn has to rely upon new sources of energy and of substrates. This switch over has to occur smoothly and any adaptation failure will result in the disturbance of impor tant metabolic functions. Mortality and morbidity in neo natal period remain still very high in comparison with the figures observed later in life; the reduction of mortality has been much more important in child and adult than in neonates.

More than for any other volume of the Handbook of Neurochemistry, the chap ters in this volume on Pathological Neurochemistry deal with the interface of the laboratory bench with the patient's bedside. But thanks to the many major advances in research and tech nology in the postwar decades, we now stand at the threshold of understanding how to manage many of the major neurological disorders, and we may expect more such delineations in the immediate decades ahead. Neurochemistry, de fined broadly, has played a central role in this extraordinary turn of events, progressing from what J. L. W, Thudichum in 1884 called objects of anxious empiricism to his anticipation of the proud exercise of chemical precision.

Growth, as we conceive it, is the study of change in an organism not yet mature. Differential growth creates form: external form through growth rates which vary from one part of the body to another and one tissue to another; and internal form through the series of time-entrained events which build up in each cell the special ized complexity of its particular function. We make no distinction, then, between growth and development, and if we have not included accounts of differentiation it is simply because we had to draw a quite arbitrary line somewhere. It is only rather recently that those involved in pediatrics and child health have come to realize that growth is the basic science peculiar to their art. It is a science which uses and incorporates the traditional disciplines of anatomy, physiology, biophysics, biochemistry, and biology. It is indeed apart ofbiology, and the study of human growth is a part of the curriculum of the rejuvenated science of Human Biology. What growth is not is aseries of charts of height and weight. Growth standards are useful and necessary, and their construction is by no means void of intellectualchallenge. Theyare a basic instrument in pediatric epidemiology. But they do not appear in this book, any more than clinical accounts of growth disorders. This appears to be the first large handbook-in three volumes-devoted to Human Growth. Smaller textbooks on the subject began to appear in the late nineteenth century, some written by pediatricians and some by anthropologists.
