In the event that the H1N1 virus creates a surge of patients during the upcoming flu season, it will be critical to protect health care workers from infection, given their central role in treating sick people and lessening the pandemic's overall impact. This new report from the Institute of Medicine recommends strategies for health care organizations and employees to prepare for the H1N1 virus. These recommendations include wearing fitted N95 respirators to guard against respiratory infection by the virus, and establishing policies for innovative triage processes, handwashing, disinfection, and more. The report also calls for a boost in research to answer questions about how the flu viruses can be spread, and to design and develop better protective equipment that would enhance workers' comfort, safety, and ability to do their jobs.

"This report updates the 2008 recommendations by CDC's Advisory Committee on Immunization Practices (ACIP) regarding the use of influenza vaccine for the prevention and control of seasonal influenza (CDC. Prevention and control of influenza: recommendations of the Advisory Committee on Immunization Practices [ACIP]. MMWR 2008;57[No. RR-7]). Information on vaccination issues related to the recently identified novel influenza A H1N1 virus will be published later in 2009. The 2009 seasonal influenza recommendations include new and updated information. Highlights of the 2009 recommendations include 1) a recommendation that annual vaccination be administered to all children aged 6 months--18 years for the 2009--10 influenza season; 2) a recommendation that vaccines containing the 2009--10 trivalent vaccine virus strains A/Brisbane/59/2007 (H1N1)-like, A/Brisbane/ 0/2007 (H3N2)-like, and B/Brisbane/60/2008-like antigens be used; and ) a notice that recommendations for influenza diagnosis and antiviral use will be published before the start of the 2009--10 influenza season. Vaccination efforts should begin as soon as vaccine is available and continue through the influenza season. Approximately 83% of the United States population is specifically recommended for annual vaccination against seasonal influenza; however,

This report updates 1999 recommendations by the Advisory Committee on Immunization Practices (ACIP) on the use of influenza vaccine and antiviral agents (MMWR 1999;48[No. RR-4]: 1-29). These recommendations include five principal changes: a) the age for universal vaccination has been lowered to 50 years from 65 years; b) scheduling of large, organized vaccination campaigns after mid-October may be considered because the availability of vaccine in any location cannot be assured consistently in the early fall; c) 2000-2001 trivalent vaccine virus strains are A/Moscow/10/99 (H3N2)-like, A/New Caledonia/20/99 (H1N1)-like, and B/Beijing/184/93-like strains; d) information on neuraminidase-inhibitor antiviral drugs has been added; and e) a list of other influenza-related infection control documents for special populations has been added. This report and other information on influenza can be accessed at the website for the Influenza Branch, Division of Viral and Rickettsial Diseases, National Center for Infectious Diseases, CDC at http://www. cdc.gov/ncidod/diseases/flu/fluvirus.htm.

The influenza virus poses a threat to human health and is responsible for global epidemics every year. In addition to seasonal infections, influenza can cause occasional pandemics of great consequence when novel viruses are introduced into humans. Despite the implementation of comprehensive vaccination programs, influenza viruses continue to pose an important and unpredictable global public health threat. They are one of the most significant causes of morbidity and mortality each year and have a significant economic impact. In recent years, research has been conducted to find alternative approaches to influenza vaccine development, including the generation of universal vaccines. Notably, significant progress in the field of influenza infection, transmission, and immunity have contributed to our understanding of influenza biology, and to expanding the technological approaches for the generation of more efficient strategies against influenza infections. Moreover, highly remarkable developments have been made in the implementation of new methodologies to evaluate the efficiency of vaccines and improve them for use on domestic animals such as poultry, horses, dogs or pigs. This enables us to decrease the exposure of humans to potentially pandemic viruses. The articles in this Special Issue will address the importance of influenza to human health and the advances in influenza research that have led to the development of better therapeutics and vaccination strategies.
