Recent decades have seen remarkable advances in the treatment of upper gastrointestinal malignancies, i.e., adenocarcinoma and squamous cell carcinoma as well as gastrointestinal stromal and other rare tumors of the esophagus and stomach. While, historically, surgical resection has been the sole treatment for these tumors, multimodal therapies have meanwhile proven their efficacy. At present, pre- and postoperative chemotherapy and radiotherapy, targeted drug therapy, and stage-specific surgical approaches are all indispensable cornerstones of an individualized treatment for upper gastrointestinal malignancies. With such multimodal treatment, better outcomes comprising improved quality of life and prolonged survival have been achieved for patients. However, for many tumor entities and stages, the ideal combination and sequence of treatments is still being evaluated in clinical trials. Moreover, the value of novel approaches such as immunotherapy or robotic surgery remains a matter of research. Gastric cancer is one of the most common and potentially lethal gastrointestinal malignancy, but it is now at the forefront of modern oncology due to new discoveries. Hepatobiliary cancers, encompassing biliary tract cancer (BTC) and hepatocellular carcinoma (HCC) are highly lethal. Biliary tract cancer is a deadly disease with a very low five-year survival rate. BTC is assumed to be the fifth most common gastrointestinal malignancy and can be categorized into extrahepatic cholangiocarcinoma (EHC), intrahepatic cholangiocarcinoma (IHC) and gallbladder cancer (GBC), based on the anatomic location. Patients suffering from BTC can be currently treated with radiation therapy, palliative or with a combination of two chemotherapeutics, cisplatin and gemcitabine. Hepatocellular carcinoma is the most prevalent form of liver cancers and was responsible for over 830,000 deaths related to cancer worldwide in 2020. HCC is therefore the second most leading cause of cancer deaths globally. Current treatment options encompass targeted therapy with sorafenib, immunotherapy and post-surgery adjuvant chemotherapy. Factors that might contribute to these dismal outcomes are diagnosis at an already late stage, due to unspecific symptoms, limited therapeutic options, lack of targets and understanding of molecular processes during carcinogenesis as well as resistance to current chemotherapy/treatment. Therefore, these current issues need to be further addressed and solutions and alternative approaches must be provided in order to detect these illnesses at an early stage, prolong the survival time of patients suffering from HCC and BTC and overcome general resistance to available treatment options. Currently, the treatment options for gastrointestinal malignancies mainly include surgery, chemotherapy, radiotherapy, and molecular targeted therapy, etc. Drug therapy is one of the main treatments for patients with advanced stages, but the efficacy of chemotherapy seems to have reached a plateau, and the progress of traditional molecular targeted therapy is relatively slow. In addition, the benefits of the current chemotherapy combined with targeted therapy for patients with advanced stages of gastrointestinal malignancies are still not satisfactory. Tumor immunotherapy is an emerging therapeutic approach and is a current research hotspot, and there are hopes that immunotherapy can help further improve the prognosis and quality of life for patients with gastrointestinal malignancies. At the same time, potential targets of immunotherapeutic drugs and prognostic biomarkers for gastrointestinal malignancies have been less studied than other common cancers, such as lung cancer. Prognostic biomarker studies are the beginning of exploring new drug targets and revealing potential mechanisms of tumor progression. Immunotherapies, particularly PD1 or PD-L1 antagonists, have demonstrated effective therapeutic efficacy against various types of cancer. To date, many PD1 drugs are available for cancer treatment, and more than 100 PD1 drugs are in clinical trials. However, the question of how to screen sensitive patients and predict the efficacy of immunotherapy remains unresolved. In addition, predictive biomarkers and treatment guidelines for immunotherapy of gastrointestinal malignancies have hardly been studied.
